摘要
目的探讨乙型肝炎病毒感染与患冠心病的相关风险性。方法将行冠状动脉造影的2974例患者根据造影结果分为冠心病组和非冠心病组,探讨HBs Ag阳性率与冠状动脉病变范围、程度的关系。结果 2974例患者中,HBs Ag阳性检出率在冠心病组和非冠心病组间差异具有统计学意义(P〈0.05),HBs Ag阳性者发生冠心病的概率是HBs Ag阴性者的1.230倍(OR=1.230,95%CI=1.092~1.385,P=0.004)。HBs Ag阳性组冠状动脉病变程度、病变支数与HBs Ag阴性组差异有统计学意义(P〈0.05)。在调整了收缩压、舒张压、肌酐、颈动脉内膜中膜厚度、HBs Ag阳性等危险因素后,HBs Ag阳性仍然是冠心病的独立危险因素,其中HBs Ag阳性时冠心病的发病危险是1.701倍(OR=1.701,95%CI=1.119~2.586,P=0.013)。结论乙型肝炎病毒感染与冠心病之间存在相关性,HBs Ag阳性者患冠心病的机率可能增加。
Aim To investigate the relationship between hepatitis B virus infection and coronary heart disease.Methods 2974 patients according to coronary angiography results were divided into coronary heart disease group and non coronary heart disease group. The relationship between HBs Ag positive rate and coronary artery lesion range and the degree was investigated. Results 2974 cases of patients,HBs Ag positive rate had statistical significance difference in coronary heart disease group and non coronary heart disease group( P 〈 0. 05). The incidence of coronary heart disease in HBs Ag positive people was 1. 230 times of HBs Ag negative people( OR = 1. 230,95% CI = 1. 092 ~ 1. 385,P =0. 004). The severity of coronary artery disease,lesion vessels had statistically significant differences in HBs Ag positive group and HBs Ag negative group( P 〈 0. 05). After adjustment of confounding factors such as systolic pressure,diastolic blood pressure,creatinine,carotid intima-media thickness( IMT) and HBs Ag positive,HBs Ag positive still was an independent risk factor of coronary heart disease. The risk of coronary heart disease was 1. 701 times when HBs Ag was positive( OR = 1. 701,95% CI = 1. 119 ~ 2. 586,P = 0. 013). Conclusion There exists correlation between hepatitis B virus infection and coronary heart disease,and the probability of HBs Ag positive people suffering from coronary heart disease may increase.
