老年慢性肺炎患者采用西医抗感染结合中医益肺汤治疗的临床效果观察

Curative Observation of Using Anti-infection Therapy Combined with Qingfei Decoction to Treat Chronic Pneumonia in Elderly People

目的：观察中医清肺汤联合西医常规治疗老年慢性肺炎的临床疗效。方法：选取我院呼吸内科2011年9月~2013年8月收治的120例老年慢性肺炎患者随机分为治疗组和对照组各60例,对照组采用常规西医抗感染治疗,治疗组在对照组的基础上联用中医方剂益肺汤治疗,比较两组患者治疗前与2周后两组患者的外周血实验室指标白细胞（WBC）、中性粒细胞、白细胞介素-2（IL-2）、γ-干扰素（IFN-γ）、白细胞介素-4（IL-4）、白细胞介素-10（IL-10）水平及疗效差异。结果：治疗组和对照组的WBC、中性粒细胞、IL-2、IFN-γ、IL-4、IL-10治疗后与治疗前比较差异均具有统计学意义（P〈0.05）;治疗后治疗组和对照组IL-2、IFN-γ、IL-4、IL-10差异均具有统计学意义（P〈0.05）。治疗组的临床疗效分布显著优于对照组且差异具有统计学意义（P〈0.05）;治疗组的总有效率91.67%高于对照组的85%但差异不具有统计学意义（P〉0.05）。结论：中医清肺汤联合西医常规治疗老年慢性肺炎较单用西医疗法具有跟显著的临床疗效。

Objective： To explore the clinical curative effect of Qingfei Decoction combined with conventional western treat- ment of elderly patients with chronic pneumonia. Methods- In our hospital from September 2011 to August 2013in department of internal medicine, 120cases of patients with chronic pneumonia patients were randomly divided into the treatment group and controlled group with 60cases in each group, the controlled group used conventional western medicine of anti infection treatment, the treatment group used Qingfei Decoction on the basis of the controlled group treatment, compared the peripheral blood white cell- laboratory index （ WBC）, neutrophil, interleukin-2 （ IL-2 ）, interferon gamma （ IFN-γ）, interleukin4 （ IL-4 ）, interleukin10 （IL-10） and the effect of different levels of two groups of patients before and after 2weeks treatment. Results： Compared with before treatment, the WBC, neutrophils, IL-2, IFN-γ, IL-4, IL-10decreased after treatment, the differences were statistically significant （P〈0. 05） ; compared with the controlled group, IL-2, IFN-γ, IL-4, IL-10of the treatment group de- creased, the difference was statistically significant （ P〈0.05 ）. The clinical curative effect of the treatment group was significant- ly better than that of the distribution of the controlled group and the difference was statistically significant （P〈0. 05 ） ; the total effective rate in treatment group was 91.67% higher than that of the controlled group of 85%, but the difference was not statisti- cally significant （ P〉0. 05 ）. Conclusion ： Qingfei Decoction Combined with routine western medicine therapy in elderly patients with chronic pneumonia is better than single western medicine therapy with remarkable clinical curative effect.
