围绝经期和绝经后子宫内膜息肉局部雌孕激素受体的表达

Expression of local estrogen receptor and progesterone receptor in endometrial polyp in patients with perimenopause and post menopause

目的：探讨围绝经期和绝经后子宫内膜息肉局部雌孕激素受体的表达情况及其在子宫内膜息肉发病中的意义。方法：选取2010年6月-2012年6月来我院妇科行宫腔镜手术术后经病理证实为子宫内膜息肉者80例,其中围绝经期38例（围绝经期组）,绝经后42例（绝经后组）,另选取同期来我院行健康体检者40例（对照组）。所有对象取内膜组织,雌激素受体（ER）和孕激素受体（PR）表达采用免疫组化SP法检测,ER和PR转录水平采用逆转录-聚合酶链反应（RT-PCR）半定量检测。结果：免疫组化结果显示,与对照组比较,围绝经期组及绝经后组ER及PR均升高明显,差异有统计学意义（P〈0.05-0.01）;与围绝经期组比较,绝经后组ER、PR升高显著（P〈0.05）。RT-PCR检测结果显示,与对照组比较,围绝经期组及绝经后组ER及PR均升高明显,差异有统计学意义（P〈0.05-0.01）;与围绝经期组比较,绝经后组ER、PR升高显著（P〈0.05）。紫外透射图表明,对照组EP、PR蛋白表达清晰,膜内外分界分明,围绝经期组及绝经后组颜色较对照组明显加重,水平较对照组均增加,细胞多呈不规则状,杂乱分布。结论：围绝经期和绝经后子宫内膜息肉的发生原因推测与激素受体局部水平的改变存在一定的关系,及时检测患者ER及PR表达水平,对于子宫内膜癌的诊断及预后判断意义重大。

Objective： To explore expression of local estrogen receptor （ER） and progesterone receptor （PR） in endom- etrial polyp in patients with perimenopause and post menopause. Methods： A total of 80 cases with confirmed endometrial pol- yp, who underwent surgery under hysteroscopy and admitted from June 2010 to June 2012 were selected. They were divided into perimenopause group （n=38） and post menopause group （n=42）, and 40 healthy persons with health examination were also selected as control group. Endometrial tissues were extracted. ER and PR protein expression was detected by immunohis- tichemical SP method, while ER and PR mRNA was detected by RT-PCR. Results;Compared with control group, protein and mRNA expressions of ER and PR in perimenopause group and post menopause group were significantly upregulated （P（0.05 or P〈0.01）, and the increase in post menopause group was more significant （P〈0.05）. ER and PR proteins in control group are clearly expressed, with clear boundary. The color was darker and the number was increased in perimenopause group and post menopause group. All proteins were of irregular shape and distributed in disorder. Conclusions：The cause of endometrial polyp is related to change of local hormone level to some extent. Detection of ER and PR in time is helpful to diagnosis and prognosis evaluation of endometrial polyp.
