疼痛护理质量指标的建立及在骨科病房的应用

Effect of pain management quality indicators on improving quality of pain management in the orthopedic department

目的探讨疼痛护理质量指标在提高骨科病房疼痛护理质量中的作用。方法运用护理质量评价指标研究方法建立疼痛护理质量评价指标,包括疼痛评估准确率、疼痛病情观察及时率、疼痛干预有效率、疼痛护理记录合格率、疼痛控制满意度。运用这5项疼痛护理质量指标对骨科病房的疼痛护理质量进行管理,实施检查监督,定期分析数据并进行持续质量改进。结果在疼痛护理质量指标管理开始实施时(2013年1月)和实施半年后(2013年6月),疼痛评估准确率分别为92.31%和95.45%,疼痛病情观察及时率分别为89.74%和97.73%,疼痛护理记录合格率分别为90.00%和97.50%,疼痛干预有效率分别87.50%和92.50%,疼痛管理满意度分别为93.33%和96.67%。同时,所有病历均记录了疼痛状况,全部均采用简易疼痛评估尺记录疼痛强度,所有患者实现定时应用止痛药物来治疗疼痛。结论将疼痛护理质量指标应用于骨科病房疼痛管理,实现了疼痛护理服务标准化、规范化,提高了患者的疼痛控制质量,为持续改进疼痛护理质量提供数据和事实依据,有助于提高疼痛护理的管理水平。

Objective To evaluate the effect of pain management quality indicators on improving the quality of pain management in orthopedic departments. Methods Pain management quality indicators were developed,which consisted of accuracy of pain assessment ,timeliness rate of pain observation ,efficiency of pain management, qualified rate of pain management records and patients＇ satisfaction with pain control. The indicators were applied in the orthopedic department and data were analyzed regularly to improve the quality of pain management continuously. Results After application of pain management quality indicators,the accuracy rate of pain assessment increased from 92.31% to 95.45%,the timeliness rate of pain observation increased from 89.74% to 97.73%,the qualified rate of pain management records increased from 90.00% to 97.50% ,the efficiency of pain management increased from 87.50% to 92.50%,and patient＇ satisfaction with pain control increased from 93.33% to 96.67%. Pain status which was assessed by pain evaluation ratio for each patient was included in medical records. Conclusion The application of pain management quality indicators can achieve the standardization and normalization of pain management,improve the quality of pain control,and provide data for continuous quality improvement of pain management.
