门冬胰岛素联合地特胰岛素与胰岛素泵对新诊断2型糖尿病疗效与安全性比较

Comparison of efficacy and safety between basal insulin detemir plus pre-meal insulin aspart and continuous subcutaneous insulin infusion in newly diagnosed type 2 diabetes

目的：比较门冬胰岛素联合地特胰岛素与胰岛素泵持续皮下输注（CSII）对新诊断2型糖尿病的疗效与安全性。方法：将56例新诊断的2型糖尿病患者随机分为门冬胰岛素联合地特胰岛素组和胰岛素泵组，治疗2周。对比2组治疗前后血糖控制情况、胰岛功能情况、血糖达标所需要的时间、胰岛素用量、低血糖发生率等情况。结果：2组治疗后空腹血糖（FPG）、空腹C肽（FC-P）、餐后2小时血糖（2hPG）、餐后2小时C肽（2hC-P）均较同组治疗前显著改善，组间比较无显著差异；2组血糖达标时间、胰岛素用量、低血糖发生率无显著差异。结论：对于新诊断的2型糖尿病患者，门冬胰岛素联合地特胰岛素皮下注射治疗，可降低患者血糖水平，改善患者的胰岛β细胞功能，与胰岛素泵同样安全、有效。

Objective：To compare the efficacy and safety between basal insulin detemir plus pre-meal insulin aspart and continuous subcutaneous insulin infusion （CSII）in newly diagnosed type 2 diabetes（T2DM）. Methods：56 newly diagnosed type 2 diabetic patients were randomized into insulin detemir plus pre-meal aspart insulin group and continuous subcutaneous insulin infusion group,and they were treated for two weeks. Blood glucose level ,β-cell function of the patients,time to target glucose,insulin dosage and hypoglycemia incidence before and after 2 weeks＇ therapy were measured and compared in both groups. Results：After the treatment,the levels of FPG and 2hPG,were significantly decreased,while the levels of FC-P and 2h C-P,were markedly increased in both groups,however,there was no statistical difference between the two groups. The time for blood glucose to the target level,the insulin dosage and hypoglycemia incidence showed no significant difference between two groups. Conclusion：For newly diagnosed type 2 diabetes,insulin detemir combined with pre-meals insulin aspart could control hyperglycemia effectively,improve the function of β-cells,having a similar efficacy and safety with CSII.
