自由体位分娩对自然分娩率和剖宫产率影响的临床观察

Effect of labor in liberal position on natural childbirth rate and cesarean section rate

目的观察自由体位分娩对提高自然分娩率、降低剖宫产率的临床效果。方法采用随机抽样的方法选取我院妇产科2012年3月至2014年3月收治的80例初产妇,依据随机数字表法将这些产妇分为研究组和对照组两组,每组平均40例。给予对照组产妇传统体位分娩护理,给予研究组产妇自由体位分娩护理。结果研究组产妇的第一产程和总产程时间均显著短于对照组（P〈0.05）,剖宫产率、阴道助产率均显著低于对照组（P〈0.05）,自然分娩率显著高于对照组（P〈0.05）,产程中安定、催产素的使用率均显著低于对照组（P〈0.05）,会阴侧切、宫颈裂伤、会阴Ⅲ度裂伤及产后出血发生率均显著低于对照组（P〈0.05）,新生儿Apgar评分≥8分比例显著高于对照组（P〈0.05）,3～7分、〈3分比例及颅内出血发生率均显著低于对照组（P〈0.05）。结论自由体位分娩能够有效提高产妇的自然分娩率、降低剖宫产率,临床效果显著,值得推广。

Objective： To observe the clinical effect of labor in liberal position on natural childbirth rate and cesarean section rate. Methods： Eighty primiparas treated in the department of gynecology and obstetrics of our hospital from March 9,012 to March 2014 were selected by random sampling method. They were randomly divided into study group and control group,40 patients for each group. The patients in the control group delivered in traditional position,while patients in the study groups labored in liberal position. Results： Compared with the control group, the first maternal labor and total labor time were significantly shorter （P 〈 0.05 ）, the natural birth rate was significantly higher （P 〈 0. 05 ）, the cesarean section rate and vaginal delivery rate were significantly lower（P〈0.05）,the amount of Diazepam, and oxytocin used in the delivery were significantly less （P 〈 0.05）, the incidence of episiotomy, cervical laceration,Ⅲ degree perineal laceration and postpartum hemorrhage were significantly lower（P〈0.05）, the proportion of neonatal Apgar score≥ 8 was significantly higher （P〈 0.05）, the proportion of Apgar score 3-7 or〈3 were significantly lower,and the incidence of intracranial hemorrhage were significantly lower（P〈0.05）in the study group. Conclusions： Labor in liberal position can effectively improve maternal natural birth rate and reduce cesarean section rate. The clinical effect is significant, so it is worthy of promotion.
