丛状血管瘤1例

A Case of Tufted Angioma

患者女,64岁。右腹股沟皮疹伴间断性疼痛和瘙痒十余年。皮肤科情况:右腹股沟可见粟粒至蚕豆大花斑状红色、暗红色斑疹,部分融合,其上及其周围有粟粒至近豌豆大红色、暗红色斑丘疹、丘疹,无压痛。皮损组织病理示:表皮大致正常,真皮内散在由紧密排列的不成熟的毛细血管组成的小叶,圆形、卵圆形、细长形或不规则形,含有较多的内皮细胞和周细胞,细胞无明显异型性,小叶周围可见扩张的淋巴管;免疫组化染色示:内皮细胞CD31和CD34表达阳性,周细胞SMA表达阳性。诊断:丛状血管瘤。

A 64-year-old female presented with skin lesions on the right groin associated with itch or pain sometimes for more than 10 years. Physical examination showed irregular granophyric, red to dark red macules with multiple,from millet to pea in size, dark reddish maculopapules on the right groin. Histopathological examination revealed nearly normal epidermis and circular, ovoid, elongated or irregularly shaped lobes composed of closely arranged immature capillaries in the dermis, including more endothelial cells and pericytes without atypia. Immunohistochemical staining showed CD31 and CD34-positive endothelial cells, and SMA-positive pericytes. The dilated lymphatic vessels around the lobes could be found. Diagnosis of Tufted Angioma was made.
