50例胸腰椎爆裂性骨折并椎管狭窄的临床疗效观察

The Observation of Clinical Effection of 50 Patients with Thoracic Lumbar Spinal Bursting Fracture and Spinal Stenosis

目的:探讨腰椎前路次切除、钛笼植入联合钢板内固定术治疗腰椎爆裂性骨折并椎管狭窄疗效。方法:选择本院2011年1月-2014年8月间收治50例患者,随机盲取法分为观察组与对照组各25例,观察组采用腰椎前路次切除、钛笼植入联合钢板内固定术治疗,对照组给予前路椎体次切除钢板内固定术治疗,观察两组手术情况、术后神经功能恢复情况。结果:观察组出院前神经功能评定,明显优于治疗前及对照组治疗后,两组比较差异有统计学意义(χ2=8.33,P<0.05)。总有效率96.00%明显高于对照组76.00%,两组比较差异有统计学意义(χ2=4.15,P<0.05)。与对照组手术时间、出血量、卧床时间、住院时间对比,差异无统计学意义(P>0.05)。结论:腰椎前路次切除、钛笼植入联合钢板内固定术治疗胸腰椎爆裂性骨折并椎管狭窄,术后疗效肯定,具有术后神经功能恢复满意等优点。

Objective：To explore the clinical effect of lumbar anterior secondary resection,titanium cage implantation combined with plate fixation for the treatment of thoracic lumbar spinal bursting fracture and spinal stenosis.Method：50 cases of patients in our hospital from January 2011 to August 2014 were collected, randomly and blindly divided them into observation group and control group,25 cases in each group,the observation group was given lumbar anterior secondary resection, titanium cage implantation combined with plate fixation for the treatment and the control group was given lumbar anterior secondary resection combined with plate fixation for the treatment,the surgery situations of the two groups as well as the nerve function recovery situation after operation were observed.Result：The nerve function of the observation group before the operation and out of the hospital was obviously better than the control group after surgery, the difference was statistically significant（χ^2=8.33,P〈0.05）.The total efficiency was 96.00% obviously better than the control group 76.00%, the difference was statistically significant（χ^2=4.15,P〈0.05）.The surgery time,postoperative blood loss,bed stayed time,and hospital stayed time were contrasted,there was no statistical significance（P〈0.05）.Conclusion：Lumbar anterior secondary resection, titanium cage implantation combined with plate fixation for the treatment of thoracic lumbar spinal bursting fracture and spinal stenosis have affirmed effect after operation as well as the nerve function recovery situation after operation.
