摘要
目的:探讨循证护理在食管癌术后颈部吻合口瘘临床护理中的应用及其实施效果。方法:选取2012年1月-2013年12月(循证护理实施后)食管癌术后颈部吻合口瘘27例,设为观察组,予以循证护理措施;回顾查找2010年1月-2011年12月(循证护理实施前)食管癌术后颈部吻合口瘘25例,设为对照组,护理措施为常规措施。比较两组的临床结局、吻合口瘘愈合时间、护理质量评分以及患者满意度。结果:除对照组1例患者中途转院治疗外,两组术后吻合口瘘痊愈率均为100%。但观察组吻合口瘘平均愈合时间短于对照组(t=4.68,P<0.05),护理质量评分高于对照组(t=4.29,P<0.05),患者满意度高于对照组(χ2=4.53,P<0.05)。结论:循证护理运用于食管癌术后颈部吻合口瘘患者的临床护理具有良好的效果,不仅细化了护理照顾措施,提高了基础护理工作质量,也增进了和谐的护患关系,患者吻合口瘘痊愈时间有效缩短,体现了护理工作的知识价值。
Objective: To explore evidence-based nursing(EBN)'s application and effect on nursing postoperative patients with cervical anastomosis fistula of esophageal cancer. Method: 27 postoperative patients with cervical anastomosis fistula of esophageal cancer from January 2012 to December 2013(after the application of EBN) were selected as observation group, and were given EBN.25 postoperative patients with cervical anastomosis fistula of esophageal cancer from January 2010 to December 2010(before the application of EBN) were selected as control group, and were given conventional nursing measures. Two groups' clinical outcome, anastomotic fistula healing time, nursing quality score and patients satisfaction were compared.Result: Except 1 patient transferred to another hospital during treatment in control group, the others' healing rate of postoperative anastomotic fistula was 100% in the two groups. Average healing time of the observation group was shorter than that of control group(t=4.68, P〈0.05), yet nursing quality score was higher than that of control group(t=4.29, P〈0.05), and patient satisfaction was higher than that of control group(χ^2=4.53, P〈0.05). Conclusion: It has good effect of EBN's application on nursing postoperative patients with cervical anastomosis fistula of esophageal cancer, not only detailings nursing care measures, improves nursing quality, but also promotes the harmonious relationship between nurses and patients, and healing time is shorten, so it embodies nursing's knowledge value.
