摘要 目的探讨胃动蛋白1(GKN1)在正常胃黏膜、癌前病变及胃癌组织中表达差异,并分析其与肿瘤部位和病理分型的关系。方法对30例胃癌患者(12例弥漫型和18例肠型),13例萎缩性胃炎患者和15例健康志愿者留取标本,并行幽门螺杆菌(Helicobacter pylori,H.pylori)检测。癌旁标本取自相应的胃癌患者。采用χ2检验分析H.pylori感染与研究对象临床病理因素之间的关系;分别采用免疫组化法和实时定量PCR法检测GKN1蛋白和GKN1 mRNA的表达,并通过单因素方差分析观察其与组织类型的关系。结果 H.pylori感染率在不同性别、肿瘤部位及病理类型中无统计学差异(P>0.05);与正常对照组和萎缩性胃炎组相比较,胃癌和癌旁组织中GKN1蛋白及mRNA表达水平显著下降(P<0.05 or P<0.01);弥漫型胃癌组织中GKN1 mRNA水平明显低于肠型胃癌组织(P<0.01)。结论 GKN1表达水平的下降可能与胃癌的发生有关,且不同病理分型中GNK1表达水平不一样。 Objective To compare the differential expression of gastrokine 1 ( GKN1 ) in normal gastric mucosa, precancerous lesions and gastric cancer tissues, and analyze its relationship with tumor site and pathological typing. Methods All biopsy specimens of mucosa were collected from 30 cases of gastric cancer( 12 cases of diffuse type and 18 ca;ses of intestinal type), 13 cases of atrophic gastritis and 15 healthy volunteers. Cancer-adjacent specimens were taken from the corresponding gastric cancer patients. Helicobaeter pylori (H. pylori ) infection was examined in all subjects. Chi-square test was used to study the relevance between H. pylori infection and the clinicopathologie features. Immunohistochemistry and real time PCR were used to detect GKN1 protein and GKN1 mRNA expression, respectively, and its relationship with tissue types was analyzed by one-way ANOVA. Results The was no significant difference with regard to H. pylori infection in different age, sex, tumor location and pathological types(P 〉0.05). GKN1 protein and mRNA expression levels in gastric cancer and precancerous lesions were significantly lower than those in normal controls and atrophic gastritis(P 〈0.05 or P 〈0.01 ). GKN1 mRNA level in diffuse gastric carcinoma was significantly lower than in intestinal gastric carcinoma (P 〈 0. 01 ). Conclusion Decrease of GKN1 expression may be associated with the occurrence of gastric cancer, and GKN1 expression level is different in different pathological types.
