口服华法林抗凝治疗永久性房颤104例及护理分析

Oral Warfarin Anticoagulation Therapy in 104 Cases of Permanent AF Patients and Analysis of Clinical Nursing

目的探讨口服华法林抗凝治疗永久性房颤临床护理体会。方法收集近年来在该医院治疗的104例应用华法林治疗的永久性房颤患者的临床资料,均采用全面护理措施,总结护理方法及护理体会。结果经卫生宣教、饮食指导、用药护理、医源性出血的预防、出血护理及出院指导等护理措施后,患者的平均华法林用量为(3.5±0.3)mg/d,INR值平均为(2.8±1.4),其中9例(8.65%)轻微出血。除1例需调整华法林服用量外,经对症处理后其他患者出血均得到控制。所有病例均未发生严重出血及严重的消化道不良反应。结论全面的临床护理对于保证口服华法林抗凝治疗永久性房颤的安全性具有重要意义。

Objective To observe the experience of clinical nursing in permanent atrial fibrillation（AF） patients treated by the oral war- farin anticoagulant therapy. Methods The clinical data in 104 cases of permanent AF treated by the warfarin anticoagulation therapy in our hospital in recent years were collected. The comprehensive nursing measures were adopted. The nursing method and experience were summarized. Results After adopting the nursing measures of health education, dietary guidance, medication care, prevention of iatrogenic bleeding, bleeding nursing measures and discharge guidance, the average warfarin use amount in this groups were （3.5 ±0. 3）mg/d, and the average INR value was （2. 8 ± 1.4）. Among them, 9 cases （8.65%） appeared mild bleeding. Except for 1 case that needed to adjust the warfarin dose, the bleeding symptoms in other cases were under control after symptomatic treatment. No serious bleeding and severe gastrointestinal adverse reactions occurred in all cases. Conclusion The comprehensive clinical nursing care has an important signifi- cance in ensuring the safety of oral warfarin anticoagulation therapy in the treatment of permanent AF.
