应用OCT检测儿童及青少年正常视网膜神经纤维层厚度 被引量:6

摘要 目的分析8-17岁儿童、青少年正常眼视网膜神经纤维层(retinal nerve fiber layer,RNFL)厚度与年龄、性别、眼别的关系,并得出其特点及参考值范围,为8-17岁儿童、青少年RNFL参数提供参考依据。方法应用Cirrus HD OCT对8-17岁儿童、青少年210人(366眼)进行以视盘为中心、直径为3.46 mm圆周的RNFL厚度测量,分别得出平均、各象限、各钟位RNFL厚度。对数据进行统计学分析,分析正常眼RNFL厚度与年龄、性别、眼别的关系,得出正常眼RNFL厚度参数参考值及其特点。结果 8-17岁儿童、青少年正常眼RNFL厚度曲线分布呈双峰型,各象限厚薄顺序为:下方〉上方〉颞侧〉鼻侧,各钟位上,6点、7点、11点、12点钟位厚度较厚,3点、4点、9点钟位厚度较薄;平均、上方、鼻侧、1点、4点钟位RNFL厚度随年龄增长而变厚,儿童、青少年组差异有统计学意义(均为P〈0.05);鼻侧、1点、2点、11点钟位RNFL厚度与眼别有关(P〈0.05);而RNFL厚度与性别无关,差异无统计学意义(P〉0.05)。结论 Cirrus HD OCT可有效测量8-17岁儿童、青少年RNFL厚度,可以为与RNFL相关的疾病提供诊断依据。 Objective To analyze the relationship between retinal nerve fiber layer thickness of the 8 -17 years old children and age,gender,different eyes,and ana- lyze its characteristics and obtain its reference value, provide reference basis of RNFL parameters for them. Methods A total of 610 eyes from 350 healthy subjects ( age ranged from 8 years to 17 years) were recruited for this study. Cirrus HD OCT was used to measure the RNFL thickness. Each subject was performed circular scans around the optic nerve with a circle size of 3.46 mm. Total average, mean quadrant and clock-hour RNFL thicknesses were recorded and compared. Reference value for normal eye RNFL thickness parametem was obtained, and the relationship between age, gender and laterality was analyzed. Results Normal RNFL thickness distribution was bimodal curve type, RNFL thickness was least for the nasal quadrant, followed by temporal, superior, inferior. RNFL thickness at 6 o' clock,7 o' clock,ll o' clock,12 o' clock were thicker, while 3 o' clock,4 o' clock,9 o' clock were thirmer;RNFL thickness at average,superior,nasal quadrant, 1 o' clock,4 o' clock were thicker with age prolong, the difference was statistically significant between child and juvenile (P 〈 0.05 ), the RNFL thickness at nasal, I o' clock, 2 o' clock, 11 o' clock were associated with different eyes (P 〈 0.05 ), there was no relationship between RNFL thickness and gender (P 〉 0.05 ). Conclusion Cirrus HD OCT is effectively used to measure the RNFL thickness of 8 - 17 years old children and provide diagnostic basis for RNFL related diseases.
