术前高压氧预处理对颈椎疾病患者神经功能恢复的影响 被引量:3

摘要 目的研究术前对颈椎疾病患者实施高压氧预处理对神经功能恢复的疗效。方法选择我院2011-08—2013-08诊治的160例具有脊髓压迫症状的脊髓型颈椎病患者,随机分为2组,对照组80例患者不实施高压氧预处理,观察组80例患在术前1周实施高压氧预处理,比较2组患者入院时、术前、术后3d以及术后6个月时日本骨科协会评估治疗分数(JOA)评分、尺神经体感诱发电位(SEP)N1、术后改善率及P1波变化情况。结果通过对2组患者实施比较,2组患者入院时以及术前1d的JOA评分差异无统计学意义;观察组患者术后3d、术后6个月的JOA评分、改善率明显优于对照组;2组患者术后3d、术后6个月的P1波、N1差异有统计学意义。结论对脊髓型脊椎病患者术前实施高压氧预处理措施,能够有效促进患者术后神经功能的恢复,值得临床推广使用。 Objective To investigate the effects of preoperative hyperbaric oxygen preconditioning on the neurological function recovery of patients with cervical vertebra disease. Methods One hundred and sixty patients with cervical spondylotic myelopathy in our hospital from August 2011 to August 2013 were randomly divided into the control group (n=80 ,treated without hyperbaric oxygen preconditioning ) and observation group (n=80 ,treated with hyperbaric oxygen preconditioning ).The Japanese Orthopaedic Association score (JOA) ,ulnar nerve somatosensory evoked potential (SEP) N1 ,postoperative im-provement rate and P1 wave of two groups on admission ,before operation ,3d and 6 months after operation were com-pared.Results The JOA score between two groups had no significant difference on admission and 1d before operation ;the JOA score and improvement rate of observation group were better than these of control group 3d and 6 months after operation ;the P1 and N1 wave between two groups had significant difference 3d and 6 months after operation. Conclusion Preoperative hy-perbaric oxygen preconditioning can effectively improve the the neurological function recovery of patients with cervical spondy-lotic myelopathy ,which is worthy of application.
