天津地区汉族人群B族Ⅰ型清道夫受体外显子8基因多态性与血清LDL-C及冠心病关系的研究 被引量:4

摘要 目的:探讨清道夫受体B族Ⅰ型基因(SR-BI)外显子8单核苷酸多态性对中国汉族人群冠心病发病易感性及血脂水平的影响。方法:应用聚合酶链反应-限制性片段长度多态性分析方法 ,测定370例冠心病患者和143例正常对照者SR-BI基因外显子8的HaeⅢ酶切基因型,分析其与冠心病、冠脉造影结果、血脂水平的关系,采用Logistic回归了解基因型与冠心病危险因素间的相互作用。结果:SR-BI基因外显子8等位基因C和T频率在冠心病组为0.809、0.191,在对照组为0.790、0.210。冠心病组与对照组比较,SR-BI外显子8 HaeⅢ酶切基因型和等位基因分布频率差异无统计学意义(P>0.05)。对照组中SR-BI基因外显子8CT+TT基因型亚组低密度脂蛋白胆固醇(LDL-C)、脂蛋白a[Lp(a)]水平显著高于同组CC基因型亚组(P<0.05)。对照组中CT+TT基因型亚组女性LDL-C、Lp(a)水平显著高于同组CC基因型亚组(P<0.05)。高血压史、糖尿病史、吸烟史、总胆固醇(TC)、LDL-C、Lp(a)为冠心病的独立危险因素,OR值分别为2.007、2.395、3.650、1.405、1.405、1.377、1.615;高密度脂蛋白胆固醇(HDL-C)、载脂蛋白AI(Apo-AI)为冠心病的保护因素,OR值分别为0.256、0.157。结论:SR-BI基因外显子8多态性可能与中国汉族人群冠心病发病易感性及冠心病病变严重程度无关,但SR-BI基因外显子8T等位基因可引起女性LDL-C和Lp(a)水平升高。 Objective:To investigate the relationship among scavenger receptor class B type I(SR-BI)single gene polymorphisms,the risk of coronary heart disease(CHD)and blood lipid concentration.Method:SR-BI exon8 polymorphisms were genotyped in 370 CHD patients and 143 healthy adults undergoing coronary angiography by PCR-RFLP.The concentrations of blood lipid were examined.Logistic regression was performed to observe the relationship between SR-BI exon 8polymorphisms and the risk factors of CHD.Result:SR-BI exon 8allelic frequencies of C and T were 0.809,0.191 in CHD group and 0.790,0.210 in control group respectively.There was no significantly difference between the distribution of HaeIII polymorphism and the allele frequency in CHD and control groups(P〉0.05).Serum levels of Low density lipoprotein-cholesterol(LDL-C)and Lipoprotein(a)[Lp(a)]in the CT+TT genotype subgroup was significantly higher than in CC genotype subgroup in control group(P〈0.05).For female,serum levels of LDL-C and Lp(a)in the CT+TT genotype subgroup was significantly higher than in CC genotype subgroup in control group(P〈0.05).The history of hypertension,diabetic mellitus and smoking,the increased levels of total cholesterol(TC),LDL-C,Lp(a)were risk factors for CHD patient(OR=2.007,2.395,3.650,1.405,1.405,1.377,1.615);the levels of high density lipoprotein-cholesterol(HDLC),apolipoprotein AI(Apo-AI)were protective factors for CHD patient(OR=0.256,0.157).Conclusion:The exon 8polymorphism of SR-BI gene may be no association with susceptibility to CHD and severity degrees of coronary lesions in Han population in China,but T allele of SR-BI exon 8can lead to the elevated LDL-C and Lp(a)concentration.
