脂肪萎缩病二例并文献复习

MRI Manifestation of Lipodistrophy Disease: Report of Two Cases with Literature Review

摘要 目的探讨脂肪萎缩病的临床和MRI表现特征,提高对该病的诊断准确率。方法搜集2例脂肪萎缩病的MRI和临床资料,复习相关文献,分析其影像学特点。结果 2例脂肪萎缩病患者均表现为全身皮下脂肪层菲薄,T1WI、T2WI脂肪信号减少或消失,1例肝、脾体积增大,肝脏和腹腔脂肪沉积,1例双侧骶骨、髂骨、坐骨脂肪沉积。临床均出现胰岛素抵抗、血糖代谢异常和高脂血症。结论脂肪萎缩病的MRI表现具有特征性,结合临床,有助于对该病的诊断。 Objective To investigate the clinical manifestations and MRI features of lipodystrophy disease in order to improve the diagnostic accuracy. Methods Two cases of lipodystrophy disease were collected. The clinical data and MRI materials were analyzed, and the relevant literatures were reviewed. Results In both cases the subcutaneous fat layer of whole body became very thin, and on T1WI and T2WI the fat signal was less or even disappeared. Hepatomegaly, spleno- megaly and fat deposition in the liver and abdomen were observed in one case; and fat deposition in bilateral sacrum, ilium and ischium were seen in another case. Clinically, insulin resistance, glucose metabolism abnormality and hyperlipidemia were seen in both cases. Conclusion The lipodystrophy disease has characteristic MRI manifestations, which is very helpful for the diagnosis when the clinical information is taken into consideration.
