子宫内膜异位症患者血清和腹腔液中VEGF、IL-6、IL-8的表达情况及临床意义 被引量:27

摘要 目的探讨血管内皮生长因子(VEGF)、白细胞介素6(IL-6)、IL-8在子宫内膜异位症(endometriosis,EMT)患者中的表达及意义。方法选取2011年1月至2013年6月我科诊治的66例EMT患者(观察组),其中Ⅱ期24例,Ⅲ、Ⅳ期42例;选取同期卵巢良性肿瘤患者60例为疾病对照组;选择健康女性42例作为正常对照组。比较各组受试者腹腔液和血清中VEGF、IL-6、IL-8水平,并对腹腔液和血清VEGF、IL-6、IL-8水平与年龄、r-AFS评分、体质指数(BMI)进行相关性分析。结果观察组腹腔液中VEGF、IL-6、IL-8含量明显高于疾病对照组,差异有统计学意义(P<0.01);观察组、疾病对照组、正常对照组血清VEGF、IL-6、IL-8水平差异有统计学意义(P<0.01);观察组血清VEGF、IL-6、IL-8水平明显高于疾病对照组和正常对照组,差异有统计学意义(P<0.01);但疾病对照组和正常对照组之间血清VEGF、IL-6、IL-8水平差异无统计学意义(P>0.05);观察组患者腹腔液VEGF、IL-6、IL-8水平高于血清中水平,差异有统计学意义(P<0.01);随着EMT分期的增加,腹腔液中VEGF、IL-6、IL-8水平呈上升趋势,差异有统计学意义(P<0.01);随着EMT分期的增加,血清VEGF、IL-6、IL-8水平呈上升趋势,差异有统计学意义(P<0.01);腹腔液和血清VEGF、IL-6、IL-8水平与r-AFS评分呈正相关(P<0.05),与年龄、BMI等因素无明显相关性(P>0.05)。结论 EMT患者腹腔液和血清中VEGF、IL-6、IL-8存在明显高表达,且随着EMT病情的进展,VEGF、IL-6、IL-8表达呈增强趋势。检测VEGF、IL-6、IL-8可以监测EMT发病及病情进展。 Objective To study the peritoneal fluid and serum VEGF, IL-6, IL-8 expression and the clinical sig-nificance of endometriosis patients. Methods From 2011 January to 2013 June, 66 EMT patients were included as the re-search subjects, of which, 24 cases were stage Ⅱ, and 42 cases were stage ⅢandⅣ. The other 60 women with benign o-varian tumor were selected as disease control group. Forty-two healthy women were used as the control group. The correla-tion between peritoneal fluid and serum levels of VEGF, IL-6, IL-8 and age, r-AFS score, BMI was analyzed. Results The peritoneal fluid VEGF, IL-6, IL-8 levels of the observation group was significantly higher than those of the benign group, the difference was significant (P&lt;0.01). There was significant difference in serum VEGF, IL-6, IL-8 levels between the observation group and benign group, as well as the control group (P&lt;0.01). Serum VEGF, IL-6 and IL-8 levels were significantly higher than those of the benign group and the control group (P〈0.01);but there was no significant difference between those of the benign group and the control group (P〈0.05). The peritoneal fluid VEGF, IL-6, IL-8 levels of the observation group was higher than that in the serum, the difference was statistically significant (P〈 0.01). With the in-crease of EMT stage, the VEGF, IL-6 and IL-8 levels in the peritoneal fluid increased significantly (P〉 0.01); and with the increase of EMT stage, the VEGF, IL-6, IL-8 level in serum tended to increase (P〈 0.01). Peritoneal fluid and serum level of VEGF, IL-6, IL-8 level and r-AFS score was positively correlated (P〈0.05), but they were not significant-ly correlated with age, BMI and other factors (P〈0.05). Conclusion Patients with EMT have peritoneal fluid and serum VEGF, IL-6, IL-8 high expression, and with the progress of EMT disease, VEGF, IL-6, IL-8 expression has shown a ten-dency of increase. VEGF, IL-6, IL-8 level can be used to monitor the pathogenesis and disease progression of EMT.
