摘要
目的:探讨电针内关穴对大鼠体外循环(CPB)后心功能的影响。方法:成年SD大鼠60只,随机分为4组(n=15):假手术组(S组)、模型组(M组)、模型+电针内关穴组(EA组)和模型+电针非穴组(EAN组)。麻醉复苏后2 h,使用超声成像系统观测左心室舒张末期内径(LVIDd)、左心室收缩末期内径(LVIDs)、短轴缩短率(FS)及左心室射血分数(LVEF)等心功能指标,然后处死动物,留取心肌组织,检测心肌组织内肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)和核因子-κB(NF-κB)的表达,光镜下HE染色观察心肌组织的病理学改变。结果:与M组比较,EA组LVIDd和LVIDs下降,FS和LVEF升高,TNF-α、IL-6和NF-κB表达减少,差异有统计学意义(P<0.05)。相关性分析显示,NF-κB的表达与TNF-α和IL-6的表达均呈正相关性(P<0.05)。HE染色显示EA组心肌细胞的形态学损伤轻于M组。结论:电针内关穴可明显改善大鼠CPB后的心功能,其机制可能与下调心肌组织NF-κB的表达,进而减少炎性细胞因子TNF-α和IL-6的生成有关。
Objective To investigate the effect of electroacupuncture at acupoint Neiguan on cardiac function in rats undergoing cardiopulmonary bypass (CPB). Methods Sixty adult Sprague-Dawley rats were randomly divided into 4 groups, 15 for each group: sham operation group (group S), model group (group M), model group with ectroacupuncture at Neiguan (group EA), and model group with electroacupuncture at non- acupoint (group EAN). Cardiac function indexes including left ventricular internal diameter at end-systole (LVIDs), left ventricular internal diastolic diameter (LVIDd), shortening fraction (FS) and left ventricular ejec- tion fraction (LVEF) were measured by ultrasound imaging system at 2 h after anesthesia resuscitation. The rats were sacrificed to obtain myocardial specimens. Expressions of tumor necrosis factor-α (TNF-α) and interleukin- 6 (IL-6) were detected by immunohistochemical staining, NF-KB expression was measured by Western blotting, and histopathological changes in myocardial tissues were observed by HE staining. Results As compared with group M, the scores of LVIDd and LVIDs were significantly lower, the scores of FS and LVEF were markedly higher, the index of myocardial apoptosis, the expressions of TNF-α, IL-6 and NF-KB were significantly de- creased in group EA (P 〈 0.05). The correlation analysis showed that NF-κB expression was positively correlated with expressions of TNF-α and IL-6 (P 〈 0.05). As compared with group M, HE staining revealed morphologi- cal injury of cardiomyocytes was alleviated in group EA. Conclusions Electroacupuncture at acupoint Neiguan can improve cardiac function in rats undergoing CPB, whose mechanism is associated with inhibition of NF-KB expression, resulting in a decrease in expressions of inflammatory cytokines TNF-α and IL-6.
