我院2012-2013年抗菌药物利用分析

Analysis of the Utilization of Antibacterial Drugs in Our Hospital during 2012-2013

目的:为抗菌药物的供应管理和指导临床合理应用提供参考。方法:从医院信息系统调取2012-2013年我院抗菌药物使用数据,采用Excel 2003软件进行统计,运用WHO推荐的限定日剂量(DDD)计算各项指标,并对采购金额、用药频度(DDDs)、日均费用(DDC)等进行回顾性汇总分析。结果:我院抗菌药物采购金额占西药采购金额的比例呈下降趋势,单环类、头霉素类、糖肽类和抗真菌类的采购金额和DDDs均有大幅下降;特殊使用级抗菌药物的采购金额和DDDs均明显下降;两年中,注射剂和口服制剂采购金额构成比分别为84.26%和15.74%、84.52%和15.48%;注射剂和口服制剂的DDDs两年的构成比分别为32.93%和67.07%、34.26%和65.74%;采购金额排序前15位的抗菌药物中,头孢地嗪排第1位,克拉霉素和头孢地尼的DDDs处于第1、2位。结论:专项整治后,我院抗菌药物临床应用趋向合理,合理用药水平得到提高,但抗菌药物的合理使用和规范化管理任重道远,必须建立长效机制。

OBJECTIVE： To provide reference for the supply management and guidance of clinical rational application of antibacterial drugs. METHODS： By retrospective study, the date of antibacterial drugs of our hospital in 2012-2013 was collected from hospital information system, DDD recommended by WHO was conduced to calculate purchase amount, and DDDs and DDC were pooled analyzed retrospectively by using Excel 2003. RESULTS： The proportion for the amount of antibacterial drugs in the amount of western medicine tended to reduce in our hospital. The amount and DDDs of monobactam, cephamycins, glycopeptides and antifungal drugs were reduced greatly. Those of antibacterial drugs for special use were also decreased significantly. The amounts of injection and oral preparation, accounted for 84.26% and 15.74%, 84.52% and 15.48%, respectively; DDDs of them accounted for 30.93% and 69.07%, 34.26% and 65.74%, respectively. Among top 15 antibacterial drugs in the list of amount, cefodizime took up the first place; and DDDs of clarithromycin and cefdinir were ranked first and second place. CONCLUSIONS： After the special rectification, the use of antibacterial drugs in our hospital tends to be rational. The rational drug use level is further improved. But the rational use and standardized management of antibacterial drugs has a long way to go. Long-term mechanism needs to be established.
