摘要 目的探讨纳米碳淋巴示踪在甲状腺癌手术中的应用价值。方法 60例甲状腺癌患者随机均分为常规组和纳米炭组,比较2组中央区淋巴结脂肪组织中误切的甲状旁腺检获情况及术后血钙、甲状旁腺素(PTH)改变情况。结果误切甲状旁腺常规组7枚(23.3%),纳米炭组0枚(0.0%)(P<0.05)。2组均无永久性低钙血症及PTH降低。一过性低钙血症常规组10例(33.3%),纳米炭组2例(6.7%)(P<0.05)。PTH暂时性减低常规组8例(26.7%),纳米炭组1例(3.3%)(P<0.05)。结论纳米碳淋巴示踪效果确切,有助于将甲状旁腺组织与甲状腺组织及淋巴脂肪组织区分开来,降低甲状腺癌淋巴清扫术中甲状旁腺的损伤概率。 Objective To explore the value of carbon nanoparticles labeled lymph node in the surgery of thyroid carcinoma. Methods Sixty patients with thyroid carcinoma were randomly divided into the routine method group and the carbon nanoparticle group. The miseut parathyroid glands in the adipose tissue of central lymph node, postoperative blood calcium and parathyroid hormone (PTH) of two groups were contrasted and analyzed. Results Seven miscut parathyroid glands (23.3%) were observed in the routine method group, while 0 (0, 0% ) in the carbon nanoparticle group ( P 〈 0.05 ). None permanent hypoealcemia and PTH decrease were observed in the two groups. There were 10 patients (33.3%) with temple hypocalcemia and 8 patients (26.7%) with temple PTH decrease respectively in the routine method group, and there were 2 patients (6.7%) and 1 patient (3.3%) respectively in the carbon nanparticle group ( P 〈 0.05 ). Conclusion The technique of carbon nanoparticles labeled lymph nodes is easy and effective, application of nanoparticle in the lymph node dissection of thyroid carcinoma can separate parahyroid glands from thyroid tissue and adipose tissue, and decrease the probability of parathyroid gland damaged.
