颈椎椎间融合术后相邻节段退变的研究进展

摘要 颈椎前路减压融合术(anterior cervical decompression and fusion,ACDF)自1958年Smith和Robinson创制以来,其疗效得到了很大肯定,给众多颈椎病患者带来福音,甚至有人将其称为治疗退行性颈椎病变"金标准"。但随着对该手术患者随访年限的延长,发现有些融合节段上下出现了不同程度相邻节段退变(adjacent segment degeneration。ASD),甚至诱发新神经症状。需再次寻求手术治疗.不但增加了患者痛苦,而且给他们带来了较大经济负担。因此.ACDF术后相邻节段退变已引起临床医生越来越多的关注.并出现了不同的解释。本研究特就其发病率、发病机制及影响因素等综述如下。
