摘要
目的观察厄贝沙坦联合黄葵胶囊治疗2型糖尿病肾病(DN)轻中度蛋白尿的临床效果。方法选取符合WH01999年颁布的糖尿病诊断标准及DN诊断标准的46例DN患者,应用随机数字表分为治疗组23例和对照组23例。对照组给予常规口服降糖药物、调脂、钙拮抗剂降压及饮食控制等治疗;治疗组在对照组常规治疗基础上,给予厄贝沙坦75mg,每日1次,黄葵胶囊5粒,每日3次,口服治疗。治疗观察时间均为8周。分别于治疗前及治疗后8周检测24h尿蛋白定量、尿素氮(BUN)、血肌酐(SCr)、C反应蛋白(CRP)等。结果治疗8周后,治疗组总有效率87.O%(20/23)明显优于对照组52.2%(12/23),差异有统计学意义(2=6.571,P〈0.05)。治疗组治疗后CRP[(11.7±O.9)mg/L降至(5.8±0.3)mg/L],相比于对照组[(10.1±0.6)mg/L降至(9.8±0.4)mg/L]显著下降,差异有统计学意义(t=4.681,P〈0.05)。结论厄贝沙坦联合黄葵胶囊可以明显降低尿蛋白,对微炎症状态也具有一定的调节作用。
Objective To observe the clinical effectof irbesartan combined with ambrette capsule fotreatmenof mild-to-moderate proteinuriin type 2 diabetinephropathy. MethodFoutty-six patientwith diabetinephropathy (DN), who meethe diagnosticriterifodiabeteand DN diagnosticriteriwhich WHO promulgated in 1999 years, were randomly divided into control group ( n = 23 ) and treatmengroup ( n = 23 )Patientin control group were administrated conventional hypoglycemidrugs, lipid, calcium antagoniststep- down and diecontrol, while in treatmengroup were administrated irbesartan (75 mg, 1 times/d) combined with ambrette capsule (5 pills, 3 times/d) on thibasis. Both the treatmenduration wa8 weeks. Urine protein, blood urenitrogen (BUN) ,serum creatinine (SCr) and reactive protein(CRP) were measured respectively before treatmenand 8 weekaftetreatment. ResultAfte8 weektreatment, the total effective rate in treatmengroup wa87.0% (20/23), highethan thaof control group(52. 2% ( 12/23 ) ;X2 = 6. 571, P 〈 0. 05 ). The CRP had decreased significantly in treatmengroup ( from ( 11.7 ±0. 9 ) mg/L to ( 5.8 ± 0. 3 ) mg/L) which compared with control group(from ( 10. 1 ±0. 6) mg/Lto (9.8 ±0. 4) mg/L;P 〈0. 05). BuBUN and Schad no significandifference compared with pre-treatmen( P 〉 0. 05 ) . Conclusion Irbesartan combine with ambrette capsule can significantly decrease the urine protein and regulate on the micro-inflammatory state. Iiworthy of popularization and application.
