摘要
目的回顾性分析吸入性损伤患者肺部损伤程度与血清胆碱酯酶(cholinesterase,CHE)值的关系。方法选取2010年1月至2014年1月在河南大学附属郑州市第一人民医院烧伤科就诊烧伤患者111例。患者分为单纯吸入性损伤组(37例)、吸入性损伤+肺部感染组(37例)、对照组(37例),比较单纯吸入性损伤组与对照组CHE情况、单纯吸入性损伤组与吸入性损伤+肺部感染组间CHE差异,并对吸入性损伤患者的CHE值与临床肺部感染评分(CPIS)情况进行相关性分析。结果吸入性损伤+肺部感染组、单纯吸入性损伤组CHE值(2 065.4±1260.3)U/L、(5 776.3±1273.9)U/L与对照组(8 117.8±1064.9)U/L比较差异有统计学意义(t=8.950、22.320,P<0.05);单纯吸入性损伤组CHE值与吸入性损伤+肺部感染组比较差异有统计学意义(t=5.803,P<0.05);单纯吸入性损伤组与吸入性损伤+肺部感染组CHE值和CPIS相关性分析系数分别为0.489、-0.713。结论吸入性损伤患者肺部损伤程度越重,其血清CHE值下降程度就越明显,二者之间存在负相关,烧伤患者血清CHE值在一定程度上可以反映其肺部病情危重程度。
Objective To retrospectively analysis on the relationship between the degree of lung injury of patients with inhalation injury and the serum cholinesterase (CHE).Methods All 111 cases of burn patients were chosen from January 2010 to January 2014 in our department,which were divided into simple inhalation injury group (37 cases),inhalation injury +lung infection group (37 cases)and control group (37 cases).The difference of CHE between the three groups of patients was compared.The value of CHE and clinical pulmonary infection score (CPIS)in patients with inhalation injury were analyzed by linear correlation.Results The value of CHE in inhalation injury + lung infection group (2 065 .4 ± 1 260.3)U/L and simple inhalation injury group (5 776.3 ±1 273.9)U/L were lower than control group (8 117.8 ±1 064.9)U/L,and the differences were statistically significant(t =8.950,22.320,P 〈0.05).The value of CHE in simple inhalation injury group and inhalation injury +lung infection group were compared,which had significant difference (t=5.803,P〈0.05).Analysis of correlation coefficient between value of CHE and CPIS in simple inhalation injury group and inhalation injury +lung infection group were -0.489,-0.718.Conclusion The more severer lung injury in inhalation injury patients,the lower serum CHE value is,which both were positively correlated.To a certain extent,the value of CHE can reflect the degree of lung injury.
