术前精确测量和评估在全髋关节置换治疗成人发育性髋关节发育不良继发骨关节炎中的价值 被引量:6

摘要 目的:探讨术前精确测量和评估在全髋关节置换治疗成人发育性髋关节发育不良(developmental dysplasia of the hip,DDH)继发骨关节炎中的价值。方法:2003年6月至2010年12月,收治25例(28髋)DDH继发骨关节炎患者,男8例,女17例。年龄25-53岁,中位数30岁。左侧发病者10例,右侧发病者12例,双侧发病者3例。病程3-250个月,中位数125个月。按照Crowe分型,Ⅰ型7例7髋、Ⅱ型10例12髋、Ⅲ型5例6髋、Ⅳ型3例3髋。行全髋关节置换术治疗,术前借助X线片和CT片进行股骨前倾角、股骨髓腔形态、髋臼骨量及截骨量的精确测量和评估,指导术中选择和植入假体,以及确定截骨量。术后随访观察患者的临床疗效。结果:本组3例患者行转子下短缩截骨,截骨长度(1.7±0.7)cm。所有患者均获随访,随访时间37-125个月,中位数76个月。未发生感染,假体脱位、松动、下沉,及假体周围骨溶解等并发症。治疗结束后,患者的疼痛视觉模拟评分由术前(6.56±0.58)分降低至(1.02±0.46)分,髋关节Harris评分由术前(49.91±7.23)分增加至(0.12±6.11)分。结论:对于采用全髋关节置换治疗的成人DDH继发骨关节炎患者,术前借助X线片和CT片进行股骨前倾角、股骨髓腔形态、髋臼骨量及截骨量的精确测量和评估,指导术中选择和植入合适的假体以及确定截骨量,有利于提高手术成功率、减轻髋关节疼痛症状及促进髋关节功能恢复。
