经皮微创股骨近端防旋髓内钉内固定治疗高龄股骨转子间骨折的手术室护理 被引量:21

摘要 目的:观察同期髌骨脱位矫正术联合全膝关节置换术治疗晚期膝骨关节炎合并习惯性髌骨脱位的临床疗效和安全性。方法:探讨经皮微创股骨近端防旋髓内钉内固定治疗高龄股骨转子间骨折的手术室护理方法。方法:2010年3月至2013年9月,对接受经皮微创股骨近端防旋髓内钉内固定术的78例高龄闭合性股骨转子间骨折患者实施了规范的手术室护理措施,男34例,女44例。年龄76-89岁,中位数80岁。按照股骨转子间骨折的Evans分型,Ⅰ型4例、Ⅱ型24例、Ⅲ型28例、Ⅳ型16例、Ⅴ型6例。合并骨质疏松症78例、高血压56例、冠心病42例、糖尿病38例。受伤至手术时间3-7 d,中位数4 d10随访观察骨折愈合及并发症发生情况。结果:手术均顺利完成。手术时间45-80 min,中位数55 min。术中出血100-260 m L,中位数150 m L。住院时间9-13 d,中位数11 d10所有患者均获得随访,随访时间3-6个月,中位数3.5个月。骨折均获得愈合。均无感染、内固定失败等并发症发生。结论:科学规范的手术室护理措施有助于手术顺利完成,缩短手术时间,提高骨折愈合率,减少术后并发症的发生。
