经皮超声造影在乳癌前哨淋巴结诊断中的应用

Experimental study in detecting sentinel lymph nodes by contrast-enhanced lymphatic ultrasonography

目的研究乳腺癌前哨淋巴结(SLN)在经皮超声造影(CEUS)显像中的影像学特征,探讨经皮CEUS在诊断乳腺癌SLN的临床应用价值。方法观察50例乳腺癌患者的术前常规CEUS图像,对有无腋窝SLN转移作出判断,并通过对术后病理标本腋窝淋巴结检测结果的随访,判断术前CEUS在诊断乳腺癌SLN转移的准确率。结果 50例中,36例(72%)成功探及SLN,14例(28%)未探及SLN,其中36例SLN判断阳性患者,31例符合术后临床病理特点,术前经皮CEUS诊断准确率为86%。CEUS判断SLN良恶性与病理诊断准确率约78%(28/36)。结论经皮CEUS术前诊断乳腺癌SLN较可靠,能协助临床医生对乳腺癌腋窝淋巴结转移情况的初步判断,有一定临床应用价值。

Objective To study the imaging features of sentinel lymph node （ SLN） by percutaneous contrast-enhanced ultrasonography （ CEUS） ,and explore the clinical value of percutaneous CEUS in diagnosis of metastatic SLN in breast carcinoma.Methods Observe the CEUS images of 50 patients with breast carcinoma,to see if the SLNs had metabasised or not,and then follow up the pathology re-sults to determine the accuracy of preoperative CEUS diagnosis of metastasis in breast cancer.Results In 50 cases,36 cases （72%） showed SLN metastasis,while 14 cases （28%） were not detected,including 36 cases positively diagnosed with SLN,and 31 cases show-ing consistent clinical pathological characteristics.Therefore,the accurate rate of preoperative percutaneous CEUS diagnostic was 86%. And accurate rate in determining malignancy of SLN via CEUS was about 78%（28/36）.Conclusion CEUS has high value in diagno-sing metastatic SLN in patients with breast carcinoma.
