老年院外带入压疮患者的管理及监控流程应用效果

The effect of outside-hospital pressure ulcer management and monitoring process implemented in elderly patients

目的探讨如何对院外带入压疮的老年患者进行更高效快捷地监控和治疗,促进压疮的愈合,提升其生活质量。方法建立健全院外带入压疮的管理制度及监控流程(以下简称"院外压疮管理及监控"),进一步贯彻执行压疮的三级管理,由科室对其进行动态观察,定期上报,护理部进行追踪监测,形成规范的报告流程。比较实施院外压疮管理及监控前半年内收治的322例、后半年内收治的282例老年患者的院外带入压疮的好转情况。结果实施前后各半年的压疮好转率分别为76.09%和86.52%,前后差异有统计学意义(P<0.05)。结论规范的院外压疮管理及监控能有效提升和保障护理工作质量,使老年患者院外带入压疮的好转率得到快速提高,从而减少患者的痛苦,大大提升老年患者的生活质量。

Objective To explore how to monitor and treat the outside-hospital pressure ulcers of elderly patients more effective and quickly,promote the healing of pressure ulcer and improve their quality of life.Methods The management system and monitoring process of outside-hospital pressure ulcers,and executed the three level management system were established.The outside-hospital pressure ulcers were dynamically observed,regularly reported.The whole process was monitored by the nursing department.The outside-hospital pressure ulcers improvement rate of elderly patients in our hospital between the management and monitoring system implemented before（2011.12-2012.5,322cases）and after（2012.6-2012.11,282cases）were compared.Results The improvement rate before and after the implementation were 76.09%and 86.52%（P〈0.05）.Conclusion The management system and monitoring process of outside-hospital pressure ulcers can effectively promote and guarantee the quality of nursing work,make the outside-hospital pressure ulcers improvement rate of elderly patients improve rapidly,reduce the pain of patients and greatly enhance the quality of life of elderly patients.
