基于文献数据分析探讨系统性红斑狼疮中医证治规律

Investigation Based on Literature Analysis about TCM Syndromes and Treatment of SLE

目的:探讨系统性红斑狼疮(SLE)的主要中医证型和用药规律,为中医药辨证论治SLE提供参考。方法:搜索近15年有关中医药和中西医结合治疗系统性红斑狼疮(SLE)的期刊文献,进行数据收集及规范化,开展描述性统计分析和聚类分析。结果:SLE以热毒炽盛、脾肾阳虚、气阴两虚、肝肾阴虚、阴虚内热为高频证候;热、阴虚、气虚、毒、阳虚、血瘀、湿、气滞为疾病的八大证素;肝、脾、肾为主要累及的脏腑;养阴、清热、化瘀、益气、解毒、凉血、温阳、利水为治疗本病的常用治法;清热药、补虚药及活血化瘀药等为使用频率较高的药类,聚类分析结果共得到4类、35味常用关系密切的药物。结论:中医辨证论治SLE以热毒炽盛、脾肾阳虚、气阴两虚、肝肾阴虚、阴虚内热为常见证候,瘀血症候常兼夹于上述5种证候当中。热毒、虚、瘀血是本病的关键病理因素,贯穿疾病各个阶段,治疗多以清热解毒、补虚及活血化瘀等立法。

Objective：To investigate the Chinese medicine major syndromes and treatment about systemic lupus erythematosus（SLE）. To provide the reference for the differential treatment. Methods：We searched SLE journal articles about Chinese medicine and integrative medicine in nearly 15 years and collected and standardized the data for descriptive statistical analysis and cluster analysis. Results：Excessive heat,spleen and kidney yang-deficiency,Qi Yin-deficiency are high-frequency syndromes in SLE. Heat,Yin-deficiency,Qi-deficiency and toxin are the mains syndrome factors. Liver,spleen and kidney are the mainly involving Zangfu-organs. Nourishing Yin,clearing heat,resolving blood stasis,replenishing Qi and detoxification are common therapeutic methods. The most frequently-used herbs included heat-clearing herbs,deficiency-nourishing drugs,blood-activating and stasis-eliminating drugs. Cluster analysis showed 4 categories and 35 commonly-used herbs. Conclusion：n Excessive heat,spleen and kidney yang-deficiency and Qi Yin-deficiency are common syndromes in Chinese medicine differential treatment about SLE and blood-stasis syndrome is frequently along with in those syndromes. The key pathological factors are toxic heats and deficiency and blood-stasis,which throughout the various stages of the disease. Heat-clearing and detoxifying,reinforcing deficiency and promoting blood circulation methods are often used to treat SLE as the major therapeutic methods.
