238例支气管肺泡灌洗液细胞病理学结果分析与临床意义 被引量:4

摘要 目的 通过对2012年3月至2014年11月贵阳市第五人民医院238例支气管肺泡灌洗液(bronchoalvelar lavage fluid,BALF)细胞病理学检查结果分析,探讨其在临床肺部疾病诊治中的意义。方法 留取支气管肺泡灌洗液15~20m L,所有标本用Thin Prep液基细胞学技术涂片后行HE染色、过碘酸-雪夫(PAS)染色、六胺银(GMS)染色、抗酸染色。结果238例BALF标本中查见异常成分的80例(33.6%);其中卡氏肺囊虫47例,分枝杆菌15例,真菌8例,巨细胞病毒2例,复合感染3例,查见肿瘤细胞5例。结论 本院送检支气管肺泡灌洗液标本异常检出率高,细胞病理学检查为肺部疾病诊断提供方便快捷的形态学依据,有重要临床应用价值。 Objective From March 2012 to November 2014 in Guiyang Fifth People's Hospital 238 cases of bronchial alveolar lavage fluid (BALF) were analyzed for cell pathology, and the clinical significance in lung disease diagnosis and treatment was explored. Methods Using 50 ml plastic tubes to collect bronchoalveolar lay- age ( 15 - 20 ml). BALF was centrifuged for 5 minutes, the supernatant was discarded and the pellets were col- lected. The ThinPrep liquid based - cytology smear technique with HE staining, periodic acid Sehiff (PAS) stai- ning, silver staining (GMS) six amine, and acid fast staining were used for analysis. Results Of 238 cases of BALF specimens, we found that 80 cases were abnormal(33.6% ) ; Of which 47 cases were Pneumocystis cari- nii, 15 cases of Mycobaeterium, 8 cases of fungal infections, 2 cases of Cytomegalovirus,3 cases of compound infection, and 5 cases of carcinoma cells. Conclusion The abnormal rate of bronchoalveolar lavage specimens was high. Cell pathology examination provides the morphological basis for fast and convenient diagnosis of pulmonary diseases. Thus BALF analysis has important clinical application values.
