妇科恶性肿瘤腹腔镜手术培训模型的建立 被引量:2

Construction of swine model about the gynecological laparoscopic malignant cancer

摘要 目的构建满意的妇科腹腔镜恶性肿瘤手术猪模型,并规范猪模型的手术步骤。方法采用全身麻醉的10头雌性猪,仰卧位,穿刺3个操作孔和1个检查孔,分别放置手术器械和腹腔镜镜头。记录子宫+输卵管卵巢切除、子宫角缝合、盆腔淋巴结切除、腹主动脉旁淋巴结切除以及膀胱修补操作的时间、失血量和并发症情况。结果共完成子宫+输卵管卵巢切除10例,子宫角缝合20例,10例盆腔淋巴结切除,10例腹主动脉旁淋巴结切除,膀胱修补10例。所有手术培训项目均顺利完成,统计学结果提示各类型手术的手术时间、术中出血量呈正态分布,无并发症发生。结论猪解剖特点可满足妇科常见类型手术的培训。使用猪作为妇科腹腔镜手术模型可以缩短手术医师适应腹腔镜手术操作过程和新器械的磨合过程。 Objective Our goal is to build up the gynecological laparoscopic swine model and to standardize surgical procedures of gynecological laparoscopic malignant tumors. Methods The study included 10 female swines. The operations were done at supine position and made three incisions for operation and one incision for monitor. We had done laparoscopic hysterectomy and tuboovariectomy, uterus suture, pelvic and para - aortic lymphadenectomy, and bladder suture. The collected data included operative time, blood loss, and incidence of complication. Results We had done 10 cases of laparoscopic hysterectomy and tuboovariectomy, 10 cases of uterus suture, 10 cases of pel- vic and para - aortic lymphadenectomy, and 10 cases of bladder suture. The statistics data indicated normal distribu- tion and no complication occurred. Conclusions The swine anatomy is suitable for gynecological laparoscopic train- ing. This model can be used to shorten the training time of gynecologist and accelerate the adaptation of new surgical instruments.
