摘要
目的:探讨中心静脉压(CVP)水平对脑外伤患者的早期疗效的影响。方法:根据格拉斯(GCS)评分将脑外伤患者分为3组:轻度组、中度组、重度组。分别将3组随机分为较低CVP值组(5~8cmH2O)、较高CVP值组(9~12cmH2O),根据瞳孔、神志、GCS评分及颅脑CT的改善,分3个时间(24h、72h和1周)观察其变化。结果:较低CVP值组的早期疗效优于较高CVP值组。结论:在患者CVP水平保持在5~8cmH2O时,既可保证患者脑灌注压,又不至于加重脑水肿,早期疗效较好。
Objective:To discuss the effects of central venous pressure (CVP) level on early efficacy of treat- ment with traumatic brain injury (TBI). Method:According to Glasgow coma scale (GCS), traumatic brain injury patients were divided into three groups:mild, moderate and severe group. The three groups were randomly divided into a lower CVP group (5~8 cmH20) and the higher CVP group (9~12 cmH20). According to the pupil con- sciousness,GCS and brain CT,to observe the changes on the three time points (24 hours, 72 hours and 1 week). Result: The lower CVP group is superior to the higher CVP group of matter the in early efficacy. Conclusion: The early efficacy of treatment is better when the CVP levels stayed around the 5 ~ 8 cmH2 O, which can ensure the cer- ebral perfusion pressure without aggravating cerebral edema.
