封闭治疗狭窄性腱鞘炎导致手指缺血1例

A case report about finger ischemia because of the closed treatment of stenosing tenosynovitis

封闭疗法是一种简单、疗效可靠的缓解急性或慢性软组织损伤、非化脓性炎症（如腰肌劳损、肩周炎、腱鞘炎、肌筋膜炎等）引起的疼痛或不适的治疗方法;但应用不当时可引起一系列并发症,甚至出现严重后果。外院一例因狭窄性腱鞘炎行封闭治疗造成手指第1、2、3指缺血坏死;我院接诊后对其进行治疗,现将诊疗体会报告如下。

Summary A 48-year-old female patient due to her right hand clenched fists outreach local pain,fatigue of ex- tract,the radial styloid process a large nodules, tenderness obviously went to the outer court closed treatment. When injection therapy after a few minutes the patient felt right 1,2,3 fingers tingling, fingers from the proximal to distal quickly pale,skin temperature decrease. About 2 hours after the skin was purple with paresthesia,but ra- dial fluctuation was good. With our vasodilator, antiplatelet agents, anticoagulants, analgesic drug, hyperbaric oxy- gen therapy the fingers were healed except for the index finger tip necrosis.
