经超声引导下注入无水乙醇及顺铂治疗卵巢巧克力囊肿的临床效果

Clinical effect on injection of anhydrous ethanol and cisplatin under the vaginal ultrasound guidance in the treatment of ovarian cysts

目的探讨经超声引导下注入无水乙醇及顺铂治疗卵巢巧克力囊肿的临床效果。方法选择本院2012年5月~2013年7月收治的60例卵巢巧克力囊肿患者作为研究对象,将其随机分为对照组与实验组,每组30例。对照组采用阴式超声引导下对巧克力囊肿穿刺单纯注入无水乙醇,实验组采用阴式超声引导下对巧克力囊肿注入无水乙醇及顺铂,比较两组治疗6、12个月的复发率及不良反应发生情况。结果实验组治疗6、12个月的复发率（6.7%、10.0%）明显低于对照组（26.7%、33.3%）,差异有统计学意义（χ2=4.32、4.81,P〈0.05）。8例患者术后出现下腹部疼痛,持续约10 min后自行缓解,其中对照组5例（16.67%）,实验组3例（10.00%）,两组腹部疼痛发生率比较,差异无统计学意义（χ2=1.57,P〉0.05）;观察组2例患者术后轻微恶心,无呕吐,1 d后症状自行消失。结论阴式超声引导下注入无水乙醇及顺铂治疗卵巢巧克力囊肿效果显著,复发率低,值得临床推广应用。

Objective To investigate clinical effect on injection of anhydrous ethanol and cisplatin under the vaginal ultrasound guidance in the treatment of ovarian cysts. Methods 60 patients with ovarian cyst from May 2012 to July2013 in our hospital were selected as observed objects and randomly divided into control group and experimental group,with 30 cases in each group.Patients of the control group were given the method of injecting anhydrous ethanol simply into the ovarian chocolate cyst by interventional puncturing under the vaginal ultrasound,the patients of experimental group were given the method of injecting anhydrous ethanol and cisplatin into the ovarian chocolate cyst,recurrence rate after treatment of 6,12 months and adverse reaction in two groups were compared. Results The recurrence rate of 6 and 12 months in experimental group（6.7%,10.0%）was lower than that in control group（26.7%,33.3%）respectively,with statistical difference（χ2=4.32,4.81,P〈0.05）.8 patients with postoperative lower abdominal pain,and spontaneous remission after 10 min,among those 8 patients,there were 5 patients（16.67%）of the control group and 3 patients（10.00%）of the experimental group,incidence rate of abdominal pain in two groups had no statistical difference（χ2=1.57,P1.57）;2patients of the observation group with postoperative mild nausea,no vomiting,spontaneous remission after one day.Conclusion The therapeutic effect on injection of anhydrous ethanol and cisplatin under the vaginal ultrasound guidance in the treatment of ovarian cysts is remarkable,with low recurrence,is worthy of clinical promotion and application.
