连续腰丛阻滞联合单次局部阻滞在人工全膝关节表面置换术后镇痛中的应用 被引量:8

摘要 目的观察连续腰丛神经阻滞联合单次局部阻滞用于单侧人工全膝关节表面置换术后镇痛的效果。方法择期行单侧人工全膝关节表面置换手术患者60例,ASAⅠ~Ⅲ级,随机分为观察组(n=30)和对照组(n=30)。观察组采用一次性便携式弹性输液系统经腰丛持续输注罗哌卡因并联合膝关节周围单次局部阻滞,对照组采用芬太尼静脉镇痛。于术后8(T1)、16(T2)、24(T3)、32(T4)、40(T5)和48 h(T6)各时点,采用疼痛VAS评分法评估患者静息状态和运动状态的疼痛程度,采用改良Bromage评分法评估下肢肌力程度,并记录术后镇痛期间两组患者恶心、呕吐、尿潴留、皮肤瘙痒、头晕等不良反应发生情况。结果观察组患者术后各观察时点静息和运动VAS评分均小于3分,且低于对照组(P〈0.05);改良Bromage下肢肌力评分两组间差异无统计学意义(P〉0.05)。观察组恶心、呕吐、尿潴留、皮肤瘙痒、头晕等不良反应发生率均低于对照组(P〈0.05)。结论连续腰丛神经阻滞联合膝关节周围单次局部阻滞用于单侧全膝关节表面置换术术后镇痛效果优于芬太尼静脉镇痛,且不良反应发生率低。
