九味镇心颗粒治疗脑卒中后焦虑性障碍患者60例疗效观察

Post Stroke Patients with Anxiety Disorder and Heart and Spleen Deficiency Syndrome Treated with Jiuwei Zhenxin Granules Observation on 60 Cases

目的观察九味镇心颗粒治疗脑卒中后焦虑性障碍的临床疗效。方法将120例心脾两虚证的脑卒中后焦虑性障碍患者随机分为治疗组和对照组各60例,治疗组口服九味镇心颗粒,每次6 g,每日3次;对照组口服氢溴酸西酞普兰片,每次20 mg,每日1次,两组均治疗6周。观察治疗前及治疗后1、2、4、6周汉密尔顿焦虑量表(HAMA)总分及其精神性焦虑与躯体性焦虑因子评分,同时进行临床疗效总评量表(CGI)、中国脑卒中临床神经功能缺损程度评分表(CSS)评分,治疗后评价临床疗效。结果治疗组临床疗效总有效率为80.67%,对照组为91.67%,两组比较差异无统计学意义(P>0.05)。两组患者治疗后4、6周HAMA、CGI、CSS评分均显著改善(P<0.05或P<0.01);两组治疗后精神性焦虑因子评分组间比较差异无统计学意义(P>0.05),而躯体性焦虑因子评分治疗组明显低于对照组(P﹤0.05)。结论九味镇心颗粒治疗脑卒中后焦虑性障碍不仅能改善患者焦虑症状,且有利于患者神经功能恢复。

Objective To observe the clinical efficacy of Jiuwei Zhenxin Granules in the treatment of post stroke anxiety disorder with heart and spleen deficiency syndrome. Methods In total 120 patients were randomly assigned to treatment group and control group,with 60 in each. The treatment group took Jiuwei Zhenxin Granules,6 g each time,three times daily,while the control group took citalopram tablets,20 mg each time,four times a day,for 6weeks. The scores of Hamilton Anxiety Scale（ HAMA）,Clinical Global Impression Scale（ CGI）,Chinese Stroke Neurological Deficit Scale（ CNS） were recorded before treatments and on week 1,2,4,and 6 after treatments to evaluate the safety and clinical efficacy. Results The total efficiency rates of the treatment group and control group were 80. 67% and 91. 67%,respectively,with no statistical significant difference（ P〉0. 05）. On week 4 and 6after treatments,scores of the HAMA and CGI were lower but CNS scores was higher in both groups compared with those before treatments（ P〈0. 05 or P〈0. 01）. After treatment,there was no significant difference in psychogenic anxiety scores between the two groups（ P〉0. 05）,but the somatic anxiety scores in the treatment group were significant lower than that in the control group（ P〈0. 05）. Conclusion Jiuwei Zhenxin Granules not only could improve the symptoms of post stroke patients with anxiety disorder and heart and spleen deficiency syndrome,but could help the recovery of neurological function and reduce the occurrence of adverse effects.
