手法配合针灸治疗老年巨大型腰椎间盘突出症的疗效研究

Clinical study on treating large type of lumbar disc herniation in old by massage plus acupuncture and moxibustion

目的:探讨手法配合针灸治疗老年巨大型腰椎间盘突出症的临床疗效,为临床上该病的治疗奠定基础。方法:选取本院2009年1月-2012年12月接收的75例老年巨大型腰椎间盘突出症患者为研究对象,按照入院顺序随机分为对照组(n=37)和观察组(n=38)。对照组给予手术治疗;观察组给予手法+针灸治疗。观察比较两组患者治疗前后VAS评分、JOA评分及屈伸、侧肌、旋转活动度。比较两组患者临床治疗效果和复发率。结果:治疗后,两组患者VAS评分等各项指标均明显改善,其中观察组改善程度显著优于对照组(P<0.05)。两组患者总有效率比较,差异无统计学意义(89.2%VS 86.8%;x2=0.098,P>0.05),观察组复发率显著低于对照组(7.9%VS 27.0%;x2=4.8,P<0.05)。结论:对于老年巨大型腰椎间盘突出症患者,手法配合针灸治疗具有良好疗效,同时能够明显改善患者疼痛和腰椎活动度,降低复发率,值得在临床上推广应用。

Objective： To investigate clinical efficacy of massage plus acupunc^tre and moxibustion on large type of lumbar disc herniation in old. Methods： 75 patients in our hospital from January 2009 to December 2012 were randomly divided into the observation group （38 cases） and the control group （37 cases） based on the order of admission. The control group was given surgery; another was given massage plus acupuncture and moxibustion. VSA scores, JOA scores, mobility of flexion and extension, pleural muscle and rotation in two groups were observe and compared before and after treatment. Clinical effects and recurrence in two groups were compared. Results： VAS scores and other indexes in two groups were obviously improved after treatment, the improving degree in the observation was better （P〈0.05）; the difference of the total efficacy in both was without statistically significant （89.2% VS 86.8%; x^2 =0.098,P〉0.05）. Conclusion： Massage plus acupuncture and moxibustion showed good efficacy on large type of lumbar disc herniation in old, could obviously improve pain and lumbar activity, reduce recurrence rate, and were worthy of clinical promotion.
