保乳手术治疗早期乳腺癌的疗效及安全性探究

Efficacy and Safety of Breast-Conserving Surgery in Early Breast Cancer

目的观察比较保乳手术与改良根治术治疗早期乳腺癌的疗效及安全性。方法选取2008年3月至2014年1月期间在我院行手术治疗的101例早期乳腺癌患者为研究对象,将其按所行术式不同随机分为研究组(53例)和对照组(48例)。研究组施行保乳手术进行治疗,对照组施行改良根治术进行治疗。比较两种术式的临床疗效和安全性。结果研究组患者的手术耗时、术中出血量、术后引流时间分别为(117.0±25.7)min,(142.4±35.0)m L,(8.2±4.6)d,对照组患者的手术耗时、术中出血量、术后引流时间分别(169.5±24.3)min,(217.5±43.1)m L,(13.1±5.3)d,研究组显著低于对照组,差异具有统计学意义(P<0.01);研究组随访期间出现局部复发、远处转移患者比例分别为7.55%和3.77%,对照组随访期间出现局部复发及远处转移患者比例分别为6.25%和8.33%,两组差异不显著,无统计学意义(P>0.05);研究组术后乳房美观效果的优良率为71.70%,对照组为0,研究组显著优于对照组,差异具有统计学意义(P<0.01)。结论保乳手术治疗早期乳腺癌在远期疗效方面与改良根治术相当,但在保持乳房美观效果方面明显优于改良根治术。

Objective To study the efficacy and safety of breast-conserving surgery in treating early breast cancer. Methods 101 cases of patients with early breast cancer treated in our hospital from March 2008 to January 2014 were selected and randomly divided into study group （53 cases） and control group （48 cases）. The study group was given with breast-conserving surgery, the control group was given with modified radical mastectomy. The clinical effectsand safety of the two groups was compared. Results The operation time, intraoperative bleeding volume, postoperativedrainage time of the study group were （117.0 ± 25.7） min, （142.4 ± 35.0） mL, （8.2 ± 4.6） d respectively, significantly less than （169.5 ± 24.3） min, （217.5 ± 43.1） mL, （13.1 ± 5.3） d of the control group, with statistical significant difference （P 〈0.01）; the incidence of local recurrence and distant metastasis of the study group were 7.55%, 3.77% respectively, the incidence of local recurrence and distant metastasis of the control group were 6.25%, 8.33% respectively, without significant difference （P 〉0.05）; the aesthetic effect of the study group was significantly better than that of the control group, with statistical siginificant difference （P 〈0.01）. Conclusions Both breast-conserving surgery and modified radical mastectomy have good long-term effect on treating early breast cancer, but the aesthetic effect of breast-conserving surgery is significantly better than that of the modified radical mastectomy.
