巨大结肠脂肪瘤一例并文献复习

Giant lipoma of the colon:a case report and review of the literature

目的探讨巨大结肠脂肪瘤的临床诊断与手术治疗效果。方法以我院近期诊治的1例及既往文献报道的8例巨大结肠脂肪瘤患者作为研究对象,分别回顾性分析病例的影像学与实验室病理检查结果,总结临床治疗方法与效果,进而判断巨大结肠脂肪瘤的临床诊疗措施。结果 9例患者中7例在术前检查中均诊断为结肠脂肪瘤,2例患者在手术病理实验后获得明确诊断。所有病例均获手术成功,术后随访复查中腹部超声与立体腹部X线片均未见异常,同时未发生切口感染、肿块残余等不良事件,均获得良好的治愈效果。结论巨大结肠脂肪瘤相对少见,影像学检查有助于诊断和鉴别诊断,最终依赖术后病理确诊,治疗以手术切除为主,预后好。

Objective To investigate the clinical diagnosis and surgical treatment of giant colon lipoma. Methods Nine cases of giant colon lipoma were retrospectively analyzed to explore the radiological and pathological features, the clinical treatment methods and results, and to evaluate the clinical diagnosis and treatment effect. Results Seven cases were diagnosed preoperatively, two cases were diagnosed after surgical pathology. All cases were successfully operated. Postoperative follow-up review with abdominal ultrasonography and abdominal plain film showed no abnormalities. No wound infection or residual tumor was occurred. Conclusion Giant lipoma is a rare colon neoplasm with low malignancy. Imaging examination is helpful for its diagnosis and differential diagnosis, final diagnosis is made by post-operation pathology.
