4种常用梅毒抗体检测方法的应用评价

Application evaluation of four common syphilis antibody detection methods

目的对4种常用梅毒抗体血清学检测方法进行试验比较,并对不同类方法展开应用评价。方法用TRUST、TP-ELISA、TPPA、胶体金法对124例梅毒阳性患者、218例对照组血清进行梅毒抗体测定,并计算灵敏度、特异性、阳性预测值、阴性预测值。结果 TRUST、TP-ELISA、TPPA、胶体金法4种方法的灵敏度、特异性分别为81.4%、96.3%;96.8%、97.7%;99.2%、99.1%;90.3%、100.0%。结论 4种方法各有其特点和适用范围。在明确诊断是否感染梅毒时,特异检测方法提供了较为特异的试验依据。而在具体的治疗过程中,可选择非特异性试验如TRUST,根据效价滴度动态变化来判断疗效。实验室应充分结合临床,根据具体情况加以选择、组合优化,合理选用不同方法进行梅毒检测,为梅毒的确诊、判断发展以及疗效提供可靠依据。

Objective To compare the four common syphilis antibody serology detection methods,and to evaluate the each application of the four different methods. Methods With toluidine red unheated serum test（ TRUST）,treponema pallidum specific antibody- enzyme linked immunosorbent assay（ TP- ELISA）,treponema pallidum particle agglutination test（ TPPA）,and colloidal gold method,serum in 124 cases of syphilis patients and in 218 cases of the control group was respectively conducted for syphilis antibodies and the sensitivity,specificity,positive predictive value and negative predictive value were respectively calculated. Results The sensitivity and specificity of TRUST,TP- ELIS,TPPA and colloidal gold method,the four methods,were respectively 81. 4%,96. 3%; 96. 8%,97. 7%; 99. 2%,99. 1%; 90. 3%,100. 0%. Conclusion The four methods had their characteristics and applicable scope. In the process of diagnosing syphilis,syphilis infection specific test method provided a more specific test basis. In the treatment of specific process,the choice of non- specific test such as TRUST,according to the titer drops of dynamic change,was used to judge the curative effect. Laboratory should be fully combined with clinical,according to the specific situation to choose,optimize combination,reasonably choose different methods for syphilis detection and provide a reliable basis for the diagnosis of syphilis,judge of syphilis＇ development and the curative effect.
