2010年-2012年嘉兴港区头孢曲松非敏感淋病奈瑟菌流行现状及penA基因研究

Study of epidemic status and Pen A gene of Neisseria gonorrhoeae isolates with nonsensitivity to ceftriaxone in Jiaxing port district from 2010 to 2012

目的调查2010年-2012年嘉兴港区头孢曲松非敏感淋病奈瑟菌的流行特征和基因型。方法收集患者中分离的淋病奈瑟菌256株,采用K-B法筛选出头孢曲松非敏感株78株,分析与头孢曲松非敏感淋病奈瑟菌感染的相关因素;用PCR扩增所有非敏感菌株的pen A基因片段,分析头孢曲松非敏感淋病奈瑟菌pen A基因的流行状况。结果在256株淋球菌中,检出头孢曲松非敏感菌株78株,占30.47%,并且每年呈上升趋势,未发现头孢曲松耐药菌株,年龄≥21岁和近2个月有服用抗生素与头孢曲松非敏感淋病奈瑟菌感染有相关。基因检测结果显示头孢曲松非敏感菌株pen A基因检出率极高,为82.05%。结论 2010年-2012年分离到的淋病奈瑟菌中头孢曲松非敏感株的比例较高,流行具有一定的特征性;本地区头孢曲松非敏感淋病奈瑟菌菌株的基因型以pen A为主。

Objective To investigate the epidemic situation and genotype of Neisseria gonorrhoeae isolates with nonsensitivity to ceftriaxone in Jiaxing port district from 2010 to 2012. Methods A total of 256 Neisseria gonorrhoeae isolates were collected,78 nonsensitive ceftriaxone strains were determined by K- B method,and the associated factors of ceftriaxone N. gonorrhoeae was analyzed; The pen A gene of all nonsensitive ceftriaxone strains was examined by PCR and analyzed their epidemic status.Results 78（ 30. 47%） showed nonsensitivity to ceflriaxone among 256 isolates,and there was increasing trend year by year,but no isolates displayed resistance to ceftriaxone,age ≥21 and antibiotic use in recent two months was associated with ceflriaxone isolates infection. Gene test results revealed that there was extremely high detected rate of pen A gene（ 82. 05%）. Conclusion The proportion of nonsensitive ceftriaxone strains among the Neisseria gonorrhoeae isolates collected from 2010 to 2012 was high. The ceflriaxone strains may have unique epidemic features in this district,and its genotype is mainly pen A gene.
