循环式腹腔热灌注化疗联合静脉化疗预防进展期胃癌术后复发的临床研究

The Clincal Study of the Prevention Value of Continuous Circulatory Hyperthermic Intraperitoneal Perfusion Combined with Intravenous Chemotherapy in Postoperative Patients with Advanced Gastric Cancer

目的探讨循环式腹腔热灌注化疗联合静脉化疗预防进展期胃癌术后复发的价值。方法2010年2月至2013年8月72例进展期胃癌患者随机分为联合组(循环式腹腔热灌注联合静脉化疗)和对照组(单纯静脉化疗),观察两组的毒副作用及疗效。结果联合组中白细胞下降(61.1%)较对照组(83.3%)发生率低,但腹痛发生率明显升高(47.2%vs13.9%);1年、2年无病生存率无区别,但腹腔复发率联合组较对照组明显降低(8.3%vs 27.8%)。结论循环式腹腔热灌注化疗联合静脉化疗能降低进展期胃癌术后腹腔复发率,且毒副反应低,值得进一步研究。

Objective To explore the circulating heat abdominal cavity perfusion chemotherapy combined intravenous chemotherapy value to prevent recurrence of advanced gastric cancer. Methods In February 2010 to August 2013, 72 cases with advanced gastric cancer patients were randomly divided into group （circulating heat abdominal cavity perfusion combined intravenous chemotherapy） and control group （simple intravenous chemotherapy）, observe the side effects of two groups and the curative effect. Results The joint group of white blood cells decline （61.1% ） was lower than those of control group （83.3 % ）, the incidence of, but the incidence of abdominal pain significantly higher （47.2% vsl3.9 % ） ; 1 year, 2 year disease - free survival, but the peritoneal recurrence joint group than the control group significantly decreased （8.3 % vs27.8 % ）. Conclusion Circulating heat abdominal cavity perfusion chemotherapy combined intravenous chemotherapy to prevent advanced gastric cancer postoperative peritoneal recurrence has certain value and low toxicity, deserves further research.
