原发性肺癌支气管动脉CTA造影分析 被引量:3

Computed Tomography Angiography of Bronchial Artery in Primary Lung Cancer

摘要 目的利用计算机断层扫描血管造影(computed tomography angiography,CTA),回顾性分析肺癌与炎症两种不同病变的病人支气管动脉(bronchial artery,BA)特征。方法收集116例患者胸部CT增强原始图像,结合横轴位图、最大密度投影(maxium intensity projection,MIP)、多平面重建(multiple planar reconstruction,MPR)、三维(three dimensional,3D)图像,进行BA重建,并测量相关数据。结果共显示BA236支。肺癌组74例,69例BA显示,共计BA170支,平均2.46支,病灶同侧显示131支;炎症对照组42例,36例见BA显示,显示BA66支,平均1.83支,其中病灶同侧显示49支;经统计学分析,肺癌组同侧与炎症组同侧BA内径无明显差异(P>0.05);肺癌组同侧总横截面较炎症组同侧BA增大(P=0.000<0.05)。结论 CTA能清楚的显示BA的形态、走行及分布,通过测量BA的内径可以为研究肺癌与炎症两种不同性质的肺部疾病中BA血流量的改变提供参考。 Objective To evaluate the image characteristics of bronchial artery with primary lung cancer and m flammation through computed tomography angiography (CTA).Methods A total of 116 cases were examined with thin- section enhanced MSCT.Three-dimensional images of bronchial arteries (BAs) were processed at the workstation.Spatial anatomical characters of the bronchial arteries were observed using volume rendering (VR) and multiplanar reconstruction (MPR)or maxium intensity projection (MIP).The difference of the morphological characteristics of bronchial artery be- tween lung cancer and inflammatory lesions were tested by analysis of variance.Results A total of 236 bronchial arteries were evaluated.A total of 170 BAs in 69 cases in lung cancer group (74 cases) was displayed with 131 BAs in the lesion side.A total of 66 BAs in 36 cases in lung Inflammation group (42 cases) was displayed with 49 BAs in the lesion side.The BA internal diameters in the ipsilateral sides of the lung cancer and inflammation were not significant ly different (P 〉 0.05) .The total cross-section areas in the ipsilateral sides of the lung cancer was increased compared with those in the in flammation group (P = 0.000(0.05).Conchlsion CTA can clearly show the morphology, course and distribution of the BAs.The BA diameter can supply indirect reference of BA blood flow in inflammation and lung cancer.
