右美托咪定预先给药对罗哌卡因所致大鼠神经毒性的影响 被引量:4

摘要 目的 :比较右美托咪定预先给药对罗哌卡因静脉注射所致的大鼠神经毒性的影响及可能机制。方法 :将80只SD大鼠随机法分为4组:对照组(C组)、罗哌卡因组(R组)、咪唑安定组(M)、右美托咪定组(D组)。C组和R组静脉缓推生理盐水10 ml/kg,10 min静脉泵注完毕;D组右美托咪定10μg/kg,容量为10 ml/kg,10 min静脉泵注完毕;M组咪唑安定0.8 mg/kg,容量为10 ml/kg,10 min静脉泵注完毕。10 min后,R组、M组和D组静脉注射1%罗哌卡因1 ml/h。以出现肢体抽搐为惊厥标准。于大鼠惊厥发作时采集血液测血浆罗哌卡因浓度,记录从罗哌卡因泵注开始到惊厥发作时间;测定脑组织谷氨酸(Glu)、天门冬氨酸(Asp)、甘氨酸(Gly)和γ-氨基丁酸(GABA)的含量。结果:与R组相比,D组及M组发生中枢神经毒性所需罗哌卡因剂量、毒性发生所需时间及血药浓度增加(P<0.05)。与C组相比,其余3组Glu、Asp、Gly、GABA的含量均增高(P<0.05);与R组比较,M组和D组Glu、Asp、Gly、GABA的含量均降低(P<0.05);与M组比较,D组的Glu的含量降低(P<0.05)。结论:预先给予右美托咪定有预防罗哌卡因所致的中枢神经毒性的作用,其机制可能为降低了中枢神经系统的兴奋性氨基酸Glu的水平。
