摘要
目的探讨细胞色素P450 2A6基因多态性与吸烟行为的关系。方法以“smok*”、“nicotine dependence”和“CYP2A6”、“eytochromeP450 2A6”、“polymorphism”、“rout*”和“varia*”为关键词,搜索PubMed、Web of Science(科学引文索引数据库)和Elsevier Science Direct中所有文献。同时以“吸烟”、“依赖”和“CYP2A6”、“细胞色素P4502A6”、“cytochromeP450 2A6”为关键词搜索中国学术文献总库(CNKI)、维普数据库(VIP)、中国生物医学文献数据库(CBM),检索范围是1999至2014年。收集关于CYP2A6基因多态性与吸烟行为的相关文献,初筛得到相关文献436篇。所有统计分析均采用Statal3.1完成。结果最终共纳入文献10篇,包括中文文献1篇,英文文献9篇。Meta分析结果显示,携带CYP2A6缺陷型等位基因者与非携带者相比,吸烟行为的开始(固定效应模型:0R=0.90,95% CI:0.78~1.03,I^2=25.8%)、吸烟行为持续性(随机效应模型:OR=0.85,95%CI:0.59~1.23,I^2=66.3%)以及戒烟(随机效应模型:DR=0.89,95%CI:0.57~1.40.I^2=67.1%)的差异均无统计学意义。而携带CYP2A6*4(CYP2A6缺失型等位基因)者开始吸烟的风险降低(OR=0.78,95%CI:0.61~0.99,I^2=28.2%),吸烟持续时间缩短(OR=0.53,95%CI:0.36~0.77,I^2=41.0%),并且更容易戒烟成功(OR=0.49,95%CI:0.31~0.80,I^2=0.0%)。结论细胞色素P450 2A6缺陷型等位基因与吸烟行为无关,而携带CYP2A6*4基因型的吸烟者开始吸烟的风险降低,吸烟持续性较差并且更容易成功戒烟。
Objective A Meta-analysis was performed to assess the association of defective hepatic cytochrome P450 2A6 (CYP2A6) gene with smoking behaviors. Methods All eligible studies published up to 2014 were searched out from PubMed, China National Knowledge lnternet (CNKI), ISI Web of knowledge (ISI), rip citation databases (VIP), Chinese BioMedical Literature (CBM) and Elsevier Science Direct, searching words were "smok * " , "nicotine dependence" , " CYP2A6" , "cytochrome P450 2A6", "polymorphism", "rout * " and "varia * ", while 436 articles were concluded. Meta-analysis was perfbrmed using Statal 3. 1. Results A total of ten studies were finally included. We didn't find a significant effect of defective CYP2A6 gene on smoking initiation ( fixed effect model ( FEM ) : OR = 0. 90, 95% CI: 0. 78 - 1.03, 12 = 25.8% ) , smoking persistence ( random effect model ( REM ) : OR = O. 85, 95%CI: 0.59- 1.23,I2 =66.3%) and smoking cessation (REM: OR=0.89, 95%CI: 0.57 - 1.40, I^2 = 67. 1% ). But it showed a significant protective effect of CYP2A6 * 4 on smoking initiation ( FEM : OR=0.78, 95%CI: 0.61 -0.99, I^2 =28.2%), smoking persistence (FEM: OR =0.53, 95% CI: 0.36-0.77, 12 =41.0%) and smoking cessation (REM: OR =0.49, 95% CI: 0.31 -0.80, I^2 = 0. 0% ). Conclusions This Meta-analysis suggested that there was not a protective effect of defective CYP2A6 gene against smoking behaviors. But smokers with whole CYP2A6 gene deletion would be less likely to start smoking, less smoking persistence and more likely to quit smoking successful than smokers with wild CYP2A6 gene.
