胫腓骨开放性骨折术后给予脱氧核苷酸联合营养支持治疗的临床有效性与安全性研究 被引量:1

摘要 目的观察脱氧核苷酸联合营养支持治疗胫腓骨开放性骨折的有效性与安全性。方法 2013年3月-2014年3月,入选我院64例胫腓骨开放性骨折患者,所有患者给予C臂X线机引导下穿针、清创、整复骨折后,装外固定架固定术后均给予抗感染治疗。治疗组术后给予脱氧核苷酸联合氨基酸、脂肪乳等支持治疗,对照组仅给予营养支持治疗,连续治疗7-14 d后评估创面愈合及感染情况。结果治疗组创面完全愈合时间,骨折延迟愈合,皮片成活面积(%),行走功能恢复时间分别为(26.7±10.4)d,0%,(98.7±1.7)%,(44.6±11.2)d。对照组分别为(30.5±10.7)d,12.5%,(91.4±2.4)%,(68.6±12.4)d。两组比较具有统计学差异(P〈0.05)。另外治疗组血清总蛋白水平,血清白蛋白,A/G水平分别为(62.2±9.4)g/L,(32.6±7.8)g/L,2.0±0.4。对照组分别为(55.3±9.2)g/L,(29.8±8.2)g/L,1.5±0.5。治疗组发生中重度感染为6.25%,对照组发生中重度感染为34.3%,两组比较差异具有统计学意义(P〈0.05)。治疗组2例出现发热。结论脱氧核苷酸治疗加速创面愈合,提高血清蛋白水平,减少感染发生。 【Objective】To observe the DNA nucleotides with nutritional support therapy efficacy and safety of the open tibiofibula fractures. 【Methods】In March, 2013- March 2014, selected from 64 patients with tibiofibula open fractures, all patients were given C arm X-ray machine guided needle, debridement, reduction of fracture, the external fixator fixing are given anti-infection therapy after surgery. Treatment group postoperatively deoxyribonucleotide joint support such as amino acid, fat emulsion treated the control group only for nutrition support treatment, continuous 7 to 14 d after treatment to evaluate wound healing and infection. 【Results】Treatment group in wound healing time completely, flap survival area(%), walking recovery time were 26.7±10.4, 98.7±1.7%, 44.6±11.2 d. The control group were 30.5±10.7, 30.5±-2.4%, 68.6±12.4 mm, two groups was statistically difference(P〈0.05). Treatment group total serum protein level, serum albumin, A/G were 62.2±9.4 G/L, 32.6±7.8 G/L, 2.0±0.4 mm. The control group were 55.3±9.2 g/L, 29.8±8.2 g/L, 1.5 ±0.5 mm. Treatment group moderately severe infection was 6.25%, the control group occurred moderately severe infection was 34.3%, the two groups was statistically difference(P〈0.05). The treatment group 2 cases of fever. 【Conclusion】DNA nucleotides therapy to accelerate wound healing, improve the level of serum protein, reduce infection.
