Ommaya储液囊植入治疗重度脑室内出血并发症分析

Complications of Ommaya reservoir implantation for severe intraventricular hemorrhage

目的探讨重度脑室出血后Ommaya囊植入术后并发症的原因、预防及处理方法。方法对海口市人民医院神经外科自2009年以来,经一侧常规脑室外引流,对侧行Ommaya储液囊植入治疗的56例重度脑室内出血病例发生的Ommaya储液囊相关并发症进行了回顾总结。结果 7例患者术后发生Ommaya储液囊相关并发症,并发症发生率为12.5%。其中Ommaya储液囊引流不畅4例,头皮感染1例,颅内感染1例,气颅1例。结论 Ommaya储液囊植入术后并发症的发生,除与患者身体状况和疾病性质有关外,还与手术操作及术后治疗等医源性因素有关。规范化手术操作及术后治疗,能有效地减少术后并发症。

【Objective】 To analyze the cause, prevention and treatment of complications after Ommaya reservoir implantation for severe intraventricular hemorrhage. 【Methods】 The complications and treatment of56 patients, who accepted external ventricular drainage and the contralateral Ommaya reservoir implantation in our hospital, were retrospectively analyzed. 【Results】 The incidence rate of complications for Ommaya reservoir implantation in the 56 patients was 12.5%（7 / 56）, including 4 cases of obstructed drainage, 1 case of scalp infection, 1 case of intracranial infection and 1case of pneumocephalus. 【Conclusion】 The complications of Ommaya reservoir implantation for severe intraventricular hemorrhage are related with the details of operations and postoperative treatment. Therefore, standard operation and postoperative treatment can effectively reduce the complications.
