肝脏瞬时弹性成像技术评价强肝化纤汤治疗慢性乙肝患者肝纤维化临床疗效研究

Clinical Effect of Liver Instantaneous Elastic Imaging Evaluation of Qianggan Huaxian Decoction in Treatment of Chronic Hepatitis Patients with Liver Fibrosis

目的:通过瞬时弹性成像技术评价强肝化纤汤治疗慢性乙肝患者肝纤维化的临床疗效。方法:将慢性乙型肝炎肝纤维化的患者随机分成实验组和对照组两组,分别对其进行肝脏瞬时弹性成像,记录弹性值后,两组均采用相同的保肝、降酶、抗病毒西药治疗,而实验组在此基础上辅以中药强肝化纤汤,治疗6个月后,对两组患者再次进行肝脏瞬时弹性成像,并记录数据,对两组患者的肝功能以及肝脏瞬时弹性成像评价纤维化程度进行比较。结果:实验组与对照组治疗前肝脏瞬时弹性成像所得弹性值差异无统计学意义(P>0.05);治疗后对两组肝脏弹性值进行比较,差异有统计学意义(P<0.05)。实验组治疗前后肝脏瞬时弹性成像所测得弹性值差异有统计学意义(P<0.05);对照组治疗前后肝脏瞬时弹性成像所测得的弹性值差异无统计学意义(P>0.05);两组治疗后肝功能比较,差异有统计学意义(P<0.05);实验组与对照组治疗后有效率分别为84.00%、69.05%,差异有统计学意义(P<0.05)。结论:通过瞬时弹性成像技术可以评价慢性肝脏纤维化程度;强肝化纤汤对慢性乙型肝炎肝纤维化患者具有一定的疗效。

Objective： To evaluate the clinical effect of Qianggan Huaxian Decoction in the treatment of chronic hepatitis B patients with liver fibrosis by liver instantaneous elastic imaging. Methods： Chronic hepatitis patients with liver fibrosis were randomly divided into experimental group and control group and both groups were respectively given the liver instantaneous elasticity imaging. After recording elasticity,two groups both used the same enzyme and protecting liver and antiviral treatment of western medicine and the experimental group was given the Qianggan Huaxian Decoction. 6 months after treatment,the patients of two groups were given instantaneous elastic again with liver imaging and we recorded and compared the data,the degree of liver function and instantaneous elasticity imaging evaluation of liver fibrosis of two groups. Results： The liver transient elastography- formed elasticity value before treatment in two groups had no statistically significant difference（ P〉0. 05）. After treatment,the liver elasticity values of two groups were compared and it had statistical significance（ P〈 0. 05）. Before and after treatment in experimental group,the liver transient elastography-formed elasticity value had statistical significance（ P〈 0. 05）. The liver transient elastography- formed elasticity value of control group before treatment and the difference had no statistical significance（ P 〉0. 05）. Liver functions of experimental group and control group before treatment were compared and the difference had no statistical significance（ P 〉0. 05）. The comparison of liver function of two groups after treatment had statistical significance（ P 〈0. 05）. The effect rates of experimental group and control group after were respectively 84. 00% and 69. 05% with statistical significance（ P 〈0. 05）. Conclusion： The instantaneous elastic imaging can evaluate chronic liver fibrosis degree and Qianggan Huaxian Decoction on patients with chronic hepatitis B liver fibrosis has certain curative effect.
