低危人群梅毒螺旋体抗体初筛试验结果分析及流程改进建议 被引量:11

摘要 目的通过分析低危人群中梅毒螺旋体抗体初筛试验结果,对相应流程提出改进措施。方法用化学发光微粒子免疫分析法(chemiluminescent microparticle immunoassay,CMIA)进行梅毒血清学初筛试验,阳性标本进行梅毒螺旋体颗粒凝集试验(Treponema pallidum particle agglutination,TPPA)和梅毒快速血浆反应素环状卡片试验(rapid plasma regain card test,RPR)。CMIA与TPPA结果不符者用荧光螺旋体抗体吸收试验(fluorescent treponemal antibody absorption test,FTA-ABS)验证。结果41 275份血清标本中,经CMIA初筛阳性608份(1.47%),CMIA S/CO值中位数(25分位数,75分位数)为7.31(2.12,20.33)。608份初筛阳性标本中,TPPA复检阳性仅369份(占60.69%),RPR阳性182份(占29.93%)。239份CMIA阳性而TPPA阴性的标本中,FTA-ABS验证结果显示,Ig M类抗体全部阴性,19份Ig G类抗体阳性。当CMIA S/CO≥1时进行TPPA法复检,TPPA复检阳性者判断为梅毒感染血清学阳性时,假阴性率为4.9%(19/388),假阳性率为0%。结论 CMIA法梅毒血清学初筛试验存在假阳性问题,应当结合TPPA复检结果进行判断,并注意定期复查。
