血清癌抗原125、甲胎蛋白、癌胚抗原联合检测对女性恶性肿瘤的诊断价值

The diagnosis value of CA125,AFP,CEA combined detection of female malignant tumors

目的:探讨CA125、AFP、CEA三种肿瘤标志物联合检测对临床常见的女性恶性肿瘤的诊断价值。方法:选取95例女性恶性肿瘤患者(恶性肿瘤组,其中子宫内膜癌45例、乳腺癌30例、卵巢癌20例),68例良性病变患者(良性病变组,疾病包括卵巢良性肿瘤、卵巢囊肿、卵巢上皮细胞瘤)及50例健康体检人群(健康对照组),对上述所有研究对象检测血清CA125、AFP、CEA。结果:恶性肿瘤组CA125、AFP、CEA水平均明显高于正常界值,也明显高于良性病变组及健康对照组(P<0.05);CA125、AFP、CEA三种肿瘤标志物单独检测对子宫内膜癌、乳腺癌及卵巢癌诊断的敏感性、特异性、准确性及PPV均较低,尤其是敏感性,维持在40%左右,PPV维持在50%左右,而三种标志物联合检测时,上述指标均有明显提高,尤其是敏感性提高最显著,提高至80%左右。结论:血清CA125、AFP、CEA三种肿瘤标志物联合检测对女性恶性肿瘤的诊断价值明显高于其单独检测,有利于早期诊断及早期治疗,改善患者预后。

Objectives： To investigate the diagnosis value of three tumor markers CA125,AFP,CEA combined detection in the common female malignant tumors. Methods： 95 female patients with malignant cancers（ malignant cancer group,including 45 cases of endometrial cancer,30 cases of breast cancer and 20 cases of ovarian cancer）,68 patients with benign lesions（ benign lesion group,including benign ovarian tumors,ovarian cysts and ovarian epithelial tumors） and 50 healthy people（ the control group） were selected to conduct serum CA125,AFP,CEA detection. Results： The CA125,AFP,CEA levels of malignant cancer group were significantly higher than normal cutoff value and these of the benign lesion group and the control group（ P〈 0. 05）; CA125,AFP,CEA alone had a low diagnostic sensitivity,specificity,accuracy and PPV in the detection of uterine endometrial cancer,breast cancer and ovarian cancer,especially the sensitivity（ about 40%） and PPV（ around 50 %）,while the com-bined detection of three markers could significantly improve these indicators,especially the sensitivity to about80%. Conclusion： The combined diagnostic value of serum CA125,AFP,CEA in the diagnosis of female cancer was significantly higher than their individual testing,which is conducive to early diagnosis and early treatment,as well as patients＇ prognosis.
