降钙素原与C反应蛋白联合检测对微创食管癌术后感染诊断的早期价值 被引量:1

摘要 目的评价血清降钙素原(PCT)、C反应蛋白(CRP)对微创食管癌患者术后早期感染的预测价值。方法将2011年9月-2014年7月行微创食管癌手术80例患者分为观察组、对照组,选择同期体检正常者30名为健康组。手术患者分别于第1、4、7天检测患者血液中的PCT、CRP,并与健康组进行比较。结果观察组和对照组术后第1天PCT、CRP与健康组比较,差异有统计学意义(P<0.05)。术后第4、7天观察组PCT、CRP与对照组比较均高于对照组,且差异有统计学意义(P<0.05)。患者的PCT、CRP敏感性和特异性分析结果表明,PCT的特异性和敏感性均高于CRP。结论微创食管癌术后并发感染可引起PCT和CRP的增高,PCT与CRP联合检测对微创食管癌术后感染的早期诊断具有临床诊断价值。 Objective To evaluate the value of procalcitonin( PCT) and C-reactive protein( CRP) on early postoperative infection in patients with minimally invasive esophagectomy. Methods Data were retrospectively analyzed on 80 cases of patients who underwent minimally invasive surgery of esophageal cancer between September 2011 to July 2014 and 30 cases of normal physical examination over the same period. All the cases were divided into an observation group,a control group and a healthy group. PCT and CRP in blood of the patients undergoing surgery were detected on the 1st,4th and 7th day after the surgery and compared with those of the healthy group. Differences among the groups were statistically analyzed using SPSS17. 0software package. Results The first day after operation,the PCT and CRP of the observation group showed statistical difference compared with the healthy group( P〈0. 05). 4 and 7 days after operation,PCT and CRP in the observation group were higher than those of the control group,and the difference was statistically significant( P〈0. 05). Analysis of sensitivity and specificity of the patients' PCT and CRP indicated that both specificity and sensitivity of PCT were higher than CRP. Conclusion Postoperative infection of minimally invasive esophagectomy is likely to be the cause of the increased PCT and CRP. The combined detection of PCT and CRP may be of clinical value to the early diagnosis of postoperative infection.
