无创呼吸机联合纳洛酮对老年COPD并发呼吸衰竭患者的临床观察

Effect of Non-invasive Ventilator and Naloxone in Treatment of Elderly COPD Patients Accompanied with Respiratory Failure

目的:探讨无创呼吸机联合纳洛酮对老年COPD并发呼吸衰竭患者的临床效果观察。方法:选取本院自2010年3月至2012年3月收治的老年COPD并发呼吸衰竭患者总共106例,采用随机分组的方法将患者分为对照组和观察组,对照组给予常规治疗和无创呼吸机应用,观察组在常规治疗和无创呼吸机应用的基础上,同时接纳纳洛酮静滴,比较两组患者治疗前后生命体征及血气分析等指标的变化情况。结果:观察组的显效率(45.3%)和总有效率(92.5%)要明显高于对照组的显效率(26.4%)和总有效率(81.1%),差别均具有统计学意义(P<0.05)。治疗后,观察组生命体征指标(RR、HR、Sa O2)、血气分析指标(Pa O2、Pa CO2、PH)要显著优于对照组,差异具有统计学意义(P<0.05)。结论:无创呼吸机联合纳洛酮在改善老年COPD伴发呼吸衰竭患者的临床症状、生命体征和血气分析指标方面具有较好的效果。

Objective：To explore the clinical effect of naloxone and non-invasive ventilator in treatment of elderly COPD patients accompanied with respiratory failure .Method： 106 cases were collected due to COPD accompany with respiratory in our hospital between March 2010 to March 2012, A randomized ap-proach were taken to divide the patients into reference group and observation group , The reference group re-ceived conventional therapy and non-invasive ventilator , while the observation group accepted naloxone infu-sion beside conventional therapy and non-invasive ventilator, indicators of RR,HR,SaO2,PaO2,pH,PaCO2 were compared before and after treatment .Result： The significant efficiency （ 45.3%） and total efficiency （92.5%） in observation group were significantly higher than the significant efficiency （26.4%） and total ef-ficiency （81.1%） in reference group （ P〈0.05）.Vital signs indicators in observation group （ RR, HR, SaO2）, blood gas analysis （PaO2, PaCO2, pH） were better than the reference group （P〈0.05）.Conclu-sion：Naloxone and non-invasive ventilator can improve clinical symptoms ,vital signs indicators and blood gas analysis indicators in elderly COPD patients accompanied with respiratory failure .
